<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.</p> <p>VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU</p> <p>VA Careers - Licensed Clinical Social Worker: https://youtube.com/embed/U_xC25QsN0w</p> <p>Total Rewards of a Allied Health Professional</p> <p>The Patient Aligned Care Team (PACT) Social Work Team Supervisor is administratively responsible for a team of approximately 5-10 PACT Social Workers who report directly to them. They are responsible for all supervisory duties for their team.</p> <ul> <li>Maintains a level of productivity and quality consistent with the complexity of the assignment and consistent with expected standards for social work professional practice, The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and VHASLCHCS. </li><li>Instructs direct reports on work expectations and encourages compliance in meeting appropriate deadlines </li><li>Monitors progress towards goals within PACT Social Work to ensure that direct reports are meeting goals and address barriers to meeting those established goals, in conjunction and consultation with Primary Care Social Work Supervisor </li><li>Performs all supervisory duties related to direct reports including, but not limited to, timekeeping, payroll, hiring, training, evaluating, managing professional practice and disciplinary actions </li><li>Acts as a liaison with the clinic leadership in their assigned primary care settings and manage the needs of assigned PACT Social Workers in assigned clinics </li><li>Plans for expected gains/losses of personnel within teams and clinics, in conjunction with the Primary Care Social Work Supervisor and service leadership. </li><li>Provides gap coverage for clinics and teams where no PACT Social Worker is assigned </li><li>Collaborates with the Veteran and the interdisciplinary care team, facilitate the delivery of health care services, understanding that the social worker is often working with Veterans who are in crisis or under stress. </li><li>Completes clinical and administrative responsibilities for the provision of social work services within their assigned team </li><li>Ensures that all social work services within assigned teams are provided within the ethical and professional guidelines of VA Social Work Professional Practice </li><li>Maintains knowledge of Veteran's benefits and services, community resources and process for making appropriate referrals to community and other governmental programs or agencies </li><li>Functions as a trainer for new PACT Social Workers as well as provide ongoing mentoring for PACT Social Workers and PACT Senior Social Workers </li><li>Establishes team goals, equitably distributes work assignments and provide general direction for the work of direct report employees. </li><li>Supports and assists clinicians in maintaining appropriate clinical records, ensuring appropriate documentation, psychosocial assessments, treatment planning and discharge plans are completed in a timely manner according to station policy </li><li>Conducts and completes psychosocial and behavioral health assessments in collaboration with the interdisciplinary treatment team, the Veteran patient, caregivers, family members and significant others, as appropriate </li><li>Conducts psychotherapeutic and psychoeducational groups to provide mental and behavioral health interventions, caregiver support, different psychosocial difficulties, loss and bereavement and group support for Veterans and/or families </li><li>Maintains an assigned PACT panel. </li></ul> <p>Work Schedule: M-F 8am -4:30pm</p> <p>EDRP Authorized: Former EDRP participants ineligible to apply for incentive. Learn more</p> <p>Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).</p> <p>Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)</p> <p>Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.</p> <p>Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.</p> <p>Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.</p> <p>Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA</p> <p>Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)</p> <p>Telework: Not available</p> <p>Virtual: This is not a virtual position.</p> <p>Functional Statement #:61072-O</p> <p>Permanent Change of Station (PCS): Not authorized</p>
