PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness.</p> <p>The ideal candidate would possess knowledge of psychiatric medications, psychiatric and medical treatment. This person would be comfortable with outreach, case management and providing support in individuals homes.</p> <p>Responsibilities:</p> <ul> <li>Act as a primary case manager to 3-6 consumers </li><li>Provide nursing interventions to individuals </li><li>Facilitate and coordinate medical care to include communication with health care providers </li><li>Provide health education to individuals </li><li>Assist in the management of medications and the overall functioning of the nursing department </li><li>Formulate and regularly update Comprehensive Assessments and IAP's for clients on your caseload </li><li>Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed. </li><li>Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clients </li></ul> <p>Qualifications:</p> <ul> <li>Requires an associates degree in nursing </li><li>LPN or RN nursing license </li><li>Experience working with adults with mental illness and knowledge of psychiatric and medical medications </li><li>Valid Massachusetts drivers license </li></ul> <p>Schedule: Monday-Friday 8am to 4pm with rotating 12pm-8pm shift , one weekend per month, rotating holiday shifts, rotating oncall schedule</p> <p>Annual Salary</p> <p>$77,680-$77,680 USD</p> <p>At Eliot, we are committed to supporting you at every stage of your career, providing opportunities for growth, purpose, and community.
