<p>A Licensed Practical Nurse to provide in-home services to our patients.</p> <ul> <li>Provides skilled nursing care ordered by the physician as indicated by the plan of care to all patients on caseload. </li><li>Review caseload with supervising RN on a regular basis as established by best practice and/or State guidelines. </li><li>Obtains necessary physician orders and collaborates with the physician as the patient's needs dictate. </li><li>Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care. </li><li>Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members (e.g. hospice aide, RN, OT, PT, MSW, ST, Dietitian, Chaplain or Hospice Manager) </li><li>Uses equipment and supplies effectively and efficiently. </li><li>Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications. </li><li>Actively participates in Quality management program. </li><li>Completes accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion. </li><li>Graduate of an accredited nursing program. </li><li>Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in state of practice. </li><li>Valid state driver's license and reliable automobile, current automobile insurance, and be willing to operate personal car necessitated by nature of job. </li><li>Experience as an LPN/LVN commensurate with one of the following preferred: </li><li>One year experience in an acute care setting within the last 24 months; </li><li>Six months home care experience within the last 12 months preferred. </li><li>Valid driver's license and reliable transportation </li><li>Proof of automobile insurance (MVR checked) </li><li>Proof of current CPR certification </li><li>Able to pass criminal background check and national sex offender clearance </li></ul>
