<p>Under the general supervision of the Community Health Manager, this position provides clinical supervision and consultation to Community Health Workers (CHWs). The role supports Ottawa Pathways to Better Health, a program designed to improve client access to medical care and social services. Key responsibilities include serving as a liaison between clients and primary care or hospital systems, gathering clinical data, and facilitating effective care coordination. The position also oversees the implementation and adherence to policies and procedures applicable to professional staff.</p> <p>This is a full-time, benefited position working out of the Public Health building located at 12251 James Street, Holland, Michigan. Travel to other Typical working hours are Monday-Friday 8:00am-5:00pm. </p> <p>The essential functions of this position include, but are not limited to, the following:</p> <ul> <li>Provides direct supervision to assigned staff, including participating in and recommending hiring, selection, and termination, overseeing performance management, training, scheduling, and work assignments. </li><li>Develops and implements Ottawa County Department of Public Health (OCDPH) and Pathways Community HUB Institute (PCHI) protocols and procedures to ensure high-quality client services, regulatory compliance, adherence to professional ethics, and fidelity to the PCHI model. </li><li>Provides training, consultation and clinical guidance to staff on Pathways workflows, community resources, and relevant health information. </li><li>Oversee CHW care coordination activities by supporting assessment of clients' medical, social, and behavioral health needs and the development and monitoring of individualized Pathways. </li><li>Monitors and evaluates CHW productivity and adjusts caseloads and workload distribution to ensure efficiency and service quality. </li><li>Collaborates with community partners, including medical providers, hospital systems, human services programs, and community organizations-to coordinate care and enhance client access to supportive services. </li><li>Collects, compiles, and analyzes program outcome and evaluation data to support continuous improvement in client experience, service delivery, and program cost-effectiveness. </li><li>Prepares and presents reports, presentations, and other communication materials for funding agencies, community partners, subcontractors, client groups, and the public. </li><li>Ensure accurate and complete case documentation and safeguards the confidentiality of client information in accordance with policy and professional standards. </li><li>Supports a diverse and inclusive work environment; and mentors and supports nursing students participating in CH programs and rotations as part of OCDPH academic partnerships. </li><li> Performs other duties as assigned. </li></ul> <p>Bachelor of Science degree in Nursing (B.S.N.) or Social Work (B.S.W.) from an accredited academic institution, and five years of progressively responsible and demonstrably successful experience in public health and/or health service delivery; or an equivalent combination of education and experience. Understanding of the Community Health Worker role and scope is preferred, and a master's degree is also preferred.</p> <p>REQUIRED KNOWLEDGE AND SKILLS:</p> <ul> <li>Thorough working knowledge of public health principles and practices, case management standards, professional boundaries, and resources that support diverse client populations. </li><li>Thorough working knowledge of managerial principles-including program planning, evaluation, contract administration, continuous quality improvement, and budget development-and demonstrated ability to model and apply strong leadership behaviors and skills. </li><li>Good working knowledge of applicable local, state, and federal regulations, including Medicaid, HIPAA, and the Michigan Public Health Code (PA 368 of 1978, as amended). </li><li>Good working knowledge of grant writing and proposal development. </li><li>Strong organizational and prioritization skills, with the ability to manage multiple tasks and competing deadlines. </li><li>Proficiency in information-technology systems, including Office 365 applications, budget and financial tracking tools, and electronic health record (EHR) platforms and documentation standards. </li><li>Ability to use data for decision-making and program improvement, supporting accurate documentation, reporting, and outcome evaluation. </li><li>Ability to interact effectively across disciplines and with individuals with varying levels of health knowledge; strong verbal, written, and presentation skills with the ability to communicate clearly with diverse audiences. </li></ul> <p>DISCLAIMER: This job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee.
