<p>EDUCATION: Graduate of an accredited Nurse Practitioner program</p> <p>LICENSURE: Valid advance practice nursing license with the state of Vermont</p> <p>OTHER: Successful completion of credentialing process for Medical Staff membership and privileges at LRHC; maintenance of active valid narcotic and controlled substances numbers and licenses.</p> <p>PHYSICAL DEMANDS: Range of motion and coordination sufficient to permit handling and lifting of patients; manual and finger dexterity, and eye-hand coordination for patient care and operation of normal compliment of physician office equipment; normal range of hearing and vision, with or without reasonable accommodation, to record and communicate with patients and co-workers effectively; occasional lifting of up to 25 pounds; ability to safely withstand exposure to communicable diseases and body fluids with appropriate use of Personal Protective Equipment; ability to assist disabled patients as necessary; ability to use appropriate body mechanics when caring for patients; ability to work effectively under emotional stress in emergency situations and during irregular hours.</p> <p>POSITION OBJECTIVE: Responsible for the medical evaluation, treatment and follow-up care of pediatric and adult primary care patients in the medical office, hospital, county prison, long-term care facility settings, and other outreach settings within the scope of practice and geographic area.</p> <p>DUTIES & RESPONSIBILITIES:</p> <p>Evaluate and treat pediatric and adult primary care patients in the medical office setting.</p> <p>Maintain written medical records in accordance with the method of practice and ensure they are available for daily review.</p> <p>Arrange referrals to physicians outside of the practice in accordance with LRHC protocols.</p> <p>The N.P. may, from standing treatment protocols or at the physician's direction, direct the dispensing of medications from the practice dispensary. The N.P. will sign prescriptions for pharmaceuticals in accordance with Article 3.1.9 of the Vermont Statutes governing Nurse Practitioner practice in VT.</p> <p>Participates in medical staff, hospital and LRHC committees.</p> <p>Maintains complete and accurate patient medical and other records with respect to services and treatment rendered to any patient, within 72 hours of provision of services.</p> <p>Instructs/mentors medical students as mutually agreed upon with Director of Clinical Operations and LRHC Administration.</p> <p>Completes insurance credentialing forms and applications in a timely manner as requested.</p> <p>Attends mandatory training and education programs as determined by LRHC.</p>
