To learn more about what the City has to offer please click here.</p> <p>Job Description:</p> <p>The City of Brockton and the towns of Abington, Avon, Stoughton, and Whitman Health Departments have formed an intermunicipal group called the Plymouth Norfolk 5 (PN5). The PN5 are engaged in an innovative initiative to strengthen core local public health services, to advance the recommendations of the 2019 Special Commission on Local and Regional Public Health and promote healthier communities across the region.</p> <p>The PN5 Shared Public Health Services group is seeking to hire a Shared Health Educator, based in Brockton, to support the Health Departments in the city of Brockton and the towns of Abington, Avon, Stoughton, and Whitman.</p> <p>SALARY: $60,000 - $70,000 / Annually</p> <p>Grant funded position. Please note the salary indicated is firm.</p> <p>Bargaining Unit: N/A</p> <p>To view the respective collective bargaining agreement please click here.</p> <p>REPORTS TO: Works under the general direction of the Shared Services Coordinator and the Plymouth Norfolk 5 board.</p> <p>SUPERVISORY RESPONSIBILITY: N/A</p> <p>ESSENTIAL FUNCTIONS:</p> <ul> <li>Develop and implement outreach plans in collaboration with team colleagues, based on individual, family, and community needs, strengths, and resources. </li><li>Identify and share appropriate information, referrals, and other resources to help residents meet their needs. </li><li>Gather and combine information from different sources to better understand residents, their families, and communities. </li><li>Initiate and sustain trusting relationships with individuals, families, social networks, and municipal staff. </li><li>Use a range of outreach methods to engage individuals and groups in diverse settings. </li><li>Share community assessment results with colleagues and community partners to inform planning and health improvement efforts. </li><li>Use effective communication skills. </li><li>Act as a cultural mediator by educating and supporting providers in working with clients from diverse cultures and help clients and community members interact effectively with professionals to promote health, improve services, and reduce health care disparities. </li><li>Addresses language and cultural barriers to care. </li><li>Coaches and guides member/representative to meet both personal and clinical goals. </li><li>Provide care coordination, which may include but not limited to facilitating care transitions, supporting the completion of referrals, and providing or confirming appropriate follow-up. </li><li>Help bridge cultural, linguistic, knowledge and literacy differences among individuals, families, communities, and providers. </li><li>Helps member/representative access community and government-based service agencies including completing paperwork for the member. </li><li>Complies with reporting, record keeping, and documentation requirements in one's work </li><li>Use appropriate technology, such as computers, for work-based communication according to municipal and Department of Public Health reporting requirements. </li><li>Creates and maintains a comprehensive inventory of local community resources, improving accessibility for patients and providers, and linking patients with the appropriate support services </li><li>Travel throughout assigned area and engage members at their homes/hospitals/community-based locations and or accompany members to appointments as appropriate </li><li>As needed, cover other areas in person or via telephonic support </li><li>Other duties as assigned. </li></ul> <p>Requirements:</p> <p>QUALIFICATIONS REQUIRED AT HIRE:</p> <p>Minimum 2-5yr experience as a Community Health Worker (CHW), Medical Assistant (MA), Engagement Specialist, Care Coordinator or Care Advocate. A high school diploma or equivalent. A valid driver's license and dependable working vehicle.</p> <p>KNOWLEDGE, SKILLS, AND ABILITIES:</p> <ul> <li>Demonstrated success in working as part of a multi-disciplinary team including communicating and working with Providers, Nurses, Social Workers, and other health care teams. </li><li>Bi/multi-lingual preferred or experience with Language Translation Services </li><li>Experience working with patients with chronic medical and behavioral health needs. </li><li>Must be flexible and adaptable to change. </li><li>Demonstrate the ability to work independently. </li><li>Must demonstrate excellent interpersonal communication skills. </li><li>Experience using appropriate technology, such as computers, for work-based communication, according to organizational requirements. </li><li>Experience and proficiency with Microsoft Office and online record keeping. </li><li>Excellent communication skills and public relations skills </li></ul> <p>PHYSICAL DEMANDS:</p> <p>Moderate physical effort is required to perform duties under typical office conditions and travel to off-site locations. The employee is frequently required to sit, speak, hear, and use hands to operate equipment. Off-site locations may include meeting spaces, schools,</p> <p>clinicians' offices, hospitals, and homes of patients/clients. Vision requirements include the ability to read and analyze documents and use a computer.</p> <p>WORK ENVIRONMENT:</p> <p>Performs administrative work in office conditions; inspection work is conducted in the field with exposure to noise and the hazardous conditions associated with construction sites requiring the use of basic safety equipment. Operates standard office equipment and an</p> <p>automobile; has frequent contact with the general public, other City employees, officials, contractors, homeowners, and builders; may have access to records of legal proceedings.
