<p>POSITION SUMMARY:</p> <p>The Community Outreach Coordinator is responsible for coordinating and conducting screening and education for the migrant/seasonal farmworkers and their families. The Community Outreach Coordinator provides literature and handouts at various locations throughout Manatee and DeSoto Counties which include, but is not limited to, farms, groceries, housing complexes, retail stores, churches, and social events.</p> <p>JOB ESSENTIAL DUTIES:</p> <ul> <li>Conducts patient education regarding healthy lifestyles in individual or group settings </li><li>Maintains logs on education sessions to include patient(s) in attendance and topics addressed </li><li>Assists providers in patient instruction </li><li>Assists in data collection for continuing quality improvement purposes </li><li>Screens and evaluates patients to determine the counseling sessions that would best suit their needs </li><li>Adheres to the safety policies of the company </li><li>Prepares education packets for distribution </li><li>Maintains demographic reports of meetings and outreach sessions which include the number of people present, ages, gender, etc. </li><li>Coordinates transportation when necessary </li><li>Develops a network with agencies serving migrant/seasonal farmworkers and their families </li><li>Participates in community events such as lectures, health fairs, and blood screenings </li><li>Translates for patients as needed </li><li>Supports the goals and mission of MCR Health Services </li><li>Adheres to the safety policy of MCR Health Services </li><li>Performs other duties as assigned from time to time </li></ul> <p>SKILLS, KNOWLEDGE & ABILITIES:</p> <ul> <li>Select and use training / instructional methods and procedures appropriate for the situation when learning or teaching new things </li><li>Ability to be flexible in schedule and adaptable to constant change </li><li>Possesses exceptional customer service skills with the ability to relate to the public and patients, regardless of ethnic, religious, and economic status </li><li>Possesses exceptional independent critical thinking skills with the ability to identify issues, collect and analyze data, establish facts, draw valid conclusions, make recommendations, and implement effective action plans to solve problems </li><li>Possess a high degree of empathy, ethics, initiative, and judgment with the ability to maintain confidence relating to sensitive matters </li></ul> <p>REQUIREMENTS & PREFERENCES:</p> <ul> <li>Minimum of High School diploma or equivalent and one year of outreach experience or community-related service preferred </li><li>Basic knowledge of Hispanic social service agencies, churches, labor camps, packing houses, farms, and apartment complexes preferred </li><li>Experience and training in providing patient education preferred </li><li>Must be able to work flexible hours totaling 40 hours per week preferred </li><li>Must have reliable transportation to travel throughout Manatee and/or DeSoto Counties </li><li>Bilingual (English/Spanish) preferred </li></ul> <p>PHYSICAL DEMANDS & EQUIPMENT USE:</p> <ul> <li>Physical demands described here represent those that must be met by an employee to successfully perform the job's essential functions.
