Reviews physician orders to determine therapeutic rationale and safe patient regimens.</p> <p>Responsible for oversight and monitoring/data collection of patients</p> <p>Educate health professionals, pharmacy staff, patients and students through in-services and written/verbal materials. Provides preceptorship and mentorship to students, faculty and residents</p> <p>Multidisciplinary collaboration for policies, procedures, guidelines and protocols</p> <p>Collaborates with specialty areas to ensure appropriate drug utilization for cost effective therapy</p> <p>Assists with cost containment initiatives through formulary review and drug use evaluations</p> <p>Provides input and supports pharmacy department initiatives in the daily collaboration with the clinical teams</p> <p>Effectively communicates recommendations and education to pharmacy and the healthcare team</p> <p>Experience Required</p> <p>3-5 years preferred.</p> <p>Education Requirements</p> <p>Pharmacy Doctorate Degree.</p> <p>License/Certification Requirements</p> <p>New Jersey Board of Pharmacy Licensure.</p> <p>Special Requirements</p> <p>Hospital pharmacy experience (or clinically relevant experience)</p> <p>PGY2 required in specialty or equivalent clinically relevant experience</p> <p>PGY1 required</p>
