<p>Develops and organizes the program of psychosocial services for children with Hem/Onc disorders and their families in the Center for Blood and Cancer Disorders (CCBD) department. Identifies needs of the patient population and plans and implements programs to meet those needs. Provides direct clinical services to patients and their families. Coordinates the multi-disciplinary team across the CCBD department of psychosocial care providers through program design, organization of the workload, supervision of social work staff, monitoring of services provided by team members, and program evaluation.</p> <p>Minimum Education</p> <p>Master's Degree Social Work or equivalent. (Required)</p> <p>Minimum Work Experience</p> <p>8 years Post Master's experience as a social worker (Required)</p> <p>5 years Post Master's experience as a social work supervisor/manager in a complex setting (Preferred)</p> <p>3 years Recent experience in a health care setting (Required)</p> <p>Required Skills/Knowledge</p> <p>Excellent oral and written communication skills.</p> <p>Ability to work effectively with persons of diverse cultural backgrounds.</p> <p>Ability to use conflict management and problem-solving skills in complex and/or emotionally charged situations.</p> <p>Understanding of psychosocial issues related to chronic illness, life-threatening illness, grief and bereavement, and the challenges of the current healthcare environment.</p> <p>Expertise in working with patients and families receiving treatment for oncologic and hematologic diseases.</p> <p>Required Licenses and Certifications</p> <p>Ind. Clinical Social Worker (Required)</p> <p>Functional Accountabilities</p> <p>Develop a comprehensive program of psychosocial services</p> <ul> <li>Provide definition and structure for support services for patients and families. </li><li>Integrate psychosocial services with primary medical and nursing services. </li><li>Promote collaboration among the psychosocial care providers. </li><li>Design tools for assessment and documentation to promote efficiency , consistency , and inter-disciplinary communication. </li><li>Coordinate the workload of psychosocial team members. </li><li>Research patient and family unmet needs and prioritizes those needs </li></ul> <p>Provide clinical services to children and their families</p> <ul> <li>Provide comprehensive psychosocial assessment for identified patients and their families. </li><li>Develop and implements a plan for intervention based on assessment. </li><li>Use range of communication skills to assist patients, families, and staff to address care needs and complex issues. </li><li>Facilitate linkage to appropriate resources in the hospital and in the community. </li><li>Intervene effectively in highly emotional or complex situations involving patients, families, and/or staff. </li><li>Provide follow-up services as appropriate and assesses outcome of interventions. </li><li>Document assessments and interventions in the patient's medical record, in accordance with hospital and departmental standards. </li></ul> <p>Ensure the quality of psychosocial services provided to patients and their families</p> <ul> <li>Convene meetings of all involved disciplines for case review, program planning, and program evaluation. </li><li>Supervise social work staff and students providing services to patients and families in the department. </li><li>Work collaboratively with non-social work members of the psychosocial team to coordinate care, monitor services provided, and resolve problems. </li><li>Design tools to assess effectiveness of services provided and patient/family satisfaction. </li></ul> <p>Collaborate with internal and external customers</p> <ul> <li>Participate actively in medical rounds and patient case conferences. </li><li>Build effective working relationships with other disciplines and departments to enhance patient care. </li><li>Facilitate resolution of customer service issues. </li><li>Support the work of Case Managers providing services to patients and the initiatives of Clinical Resource Management. </li><li>Offer formal and informal presentations about psychosocial issues for this population. </li><li>Organize family members and other interested friends to develop an Advisory Group to offer additional services and assistance. </li><li>Serve as liaison to community organizations involved with this population. </li></ul> <p>Organizational Accountabilities</p> <p>Deliver</p> <ul> <li>Set and clearly communicate team goals and priorities in alignment with departmental goals and budgets </li><li>Develop the budget and assign resources to meet the team goals </li><li>Provide the resources and guidance required for employees to perform effectively </li><li>Develop procedures to ensure high safety and quality, and course-correct as needed </li><li>Identify customers' needs and ensure service excellence in meeting those needs </li></ul> <p>Engage</p> <ul> <li>Be the link between the department and the team in defining the strategies to meet team goals </li><li>Provide prompt and clear feedback to staff and support their performance </li><li>Ensure team adherence to organizational regulations </li><li>Manage the working environment to promote productivity and motivation </li><li>Represent the team in clearing obstacles to high performance </li><li>Hire staff and develop their capabilities </li><li>Monitor and promote strong employee engagement </li></ul> <p>Plan for program development</p> <ul> <li>Work with the psychosocial team to identify needs for programs and services as well as service innovations. </li><li>Design programs to meet identified needs. </li><li>Integrate psychosocial planning with new initiatives within the department/division/center . </li><li>Seek opportunities for new programs, changes in service delivery, and community outreach. </li><li>Investigate resources to fund and staff new or expanded programs. </li><li>Establish partnerships with interested individuals and community groups to develop or enhance patient/family support programs. </li><li>Work effectively in securing external funding sources to sustain and expand the program. </li><li>Monitor appropriateness and effectiveness of new programs. </li></ul> <p>Organizational Accountabilities (Staff)</p> <p>Organizational Commitment/Identification</p> <p>Teamwork/Communication</p> <p>Performance Improvement/Problem-solving</p> <p>Cost Management/Financial Responsibility</p> <p>Safety</p>
