<p>SERVICE NAVIGATOR</p> <p>29050</p> <p>(Competitive)</p> <p>DISTINGUISHING FEATURES OF THE CLASS</p> <p>The work involves responsibility for performing outreach, support and referrals for individuals experiencing substance use disorders. An employee in this class is responsible for completing direct advocacy and referrals for individuals in need of supportive services. Certain assignments made to employees in this class will require reasonable access to transportation to meet field work requirements made in the ordinary course of business in a timely and efficient manner. Direct supervision is received from a Program Coordinator. Supervision is not a function of this class. Does related work as required.</p> <p>TYPICAL WORK ACTIVITIES</p> <p>Contacts potential participants and referrals in a timely manner.</p> <p>Conducts street and community outreach to individuals who might not otherwise seek assistance for issues related to substance use.</p> <p>Connects individuals and families to identified substance use supports and services.</p> <p>Acts as a referral agent for patients to other Health Department and community-based programs and services.</p> <p>Assists participants in understanding and accessing available community supports and resources.</p> <p>Collaborates with care managers and patient navigators from other community agencies.</p> <p>Maintains contact with clients, including phone contacts and field visits.</p> <p>Conducts outreach efforts to participants who have been disengaged from services.</p> <p>Completes required documentation, including tracking mechanisms progress notes and care plans, in accordance with agency standards.</p> <p>Attends and participates in monthly staff meetings.</p> <p>Participates in public health preparedness activities as trained and assigned.</p> <p>Utilizes appropriate methods for interacting effectively and professionally with persons of all ages and from diverse cultural, socioeconomic, educational, racial and ethnic backgrounds, sexual orientations, lifestyles and physical abilities.</p> <p>FULL PERFORMANCE KNOWLEDGES, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS</p> <p>Good knowledge of local agencies and Health Department services.</p> <p>Ability to assist individuals and families with navigating the Substance Use Services system.</p> <p>Ability to understand and interpret written material.</p> <p>Ability to assess needs and develop a person-centered plan with the client.</p> <p>Ability to establish effective working relationships with health professionals, program participants and clients.</p> <p>Ability to collaborate with other service providers to deliver person-centered services.</p> <p>Ability to follow oral and written instructions.</p> <p>Ability to teach life skills.</p> <p>MINIMUM QUALIFICATIONS</p> <p>Open Competitive</p> <p>Candidates must be high school graduates or holders of a high school equivalency diploma issued by an education department of any of the states of the United States or holders of a comparable diploma issued by any territory, commonwealth, or possession of the United States or by the Canal Zone or holders of a report from the United States Armed Forces certifying successful completion of the tests related to general education development, high school level.</p> <p>Special Necessary Requirement:</p> <p>Must have a valid NYS driver's license appropriate to the vehicles being operated or otherwise demonstrate the ability to meet the field requirements of the position.</p> <p>01/2025 Date of Original Composition</p>
