<p>Eden Home Health is hiring an RN Director of Patient Care Services or RN Clinical Manager to oversee and provide in-home services to our patients throughout Sacramento, CA.</p> <p>Why choose Eden? In addition, Eden Health is a Certified Great Place to Work company, 6 years in a row!</p> <p>Job Summary:</p> <ul> <li>Responsible for developing, planning, organizing, and directing patient care services in </li></ul> <p>accordance with federal and state regulations, accrediting bodies, and recognized standards</p> <p>of care.</p> <ul> <li>Implements policies and procedures; goals and objectives; and standards of care relevant to </li></ul> <p>patient services and staff functions.</p> <ul> <li>Maintains staffing levels as needed to provide patient care to meet changing demands in </li></ul> <p>patient acuity and admission volumes. Guarantees schedules provide proper coverage for</p> <p>patient care needs.</p> <ul> <li>Supports and maintains a culture of safety and quality. </li><li>Directs and supports the Quality Assurance/Performance Improvement (QAPI) activities, </li></ul> <p>including evaluation, planning, and implementation.</p> <ul> <li>Assists in preparing and meeting financial targets set by the Agency's annual budget. </li></ul> <p>Contributes to the ongoing budget management and gathering of financial data in</p> <p>conjunction with the Executive Director.</p> <ul> <li>Aids in the Agency's goal of retaining quality staff by appropriately hiring, orienting, training, </li></ul> <p>evaluating, and disciplining staff, including contract staff, within established policy. Assists in</p> <p>the development and implementation of orientation and ongoing training programs provided</p> <p>for Agency staff.</p> <ul> <li>Assists in establishing criteria for the admission and discharge of patients. </li><li>Is readily available during operating hours. </li><li>Maintains a working knowledge of current Medicare, Medicaid, and third-party payer </li></ul> <p>requirements for home health reimbursement. Possesses understanding of applicable state</p> <p>and federal regulations as applicable to home health and communicates updates to staff.</p> <ul> <li>Works closely and communicates with Executive Director to ensure highest quality of care to </li></ul> <p>patients, efficient operations, business development, and financial stewardship.</p> <ul> <li>Communicates and coordinates with physician's offices, payers, patients/families, outpatient </li></ul> <p>services and inpatient coordinators, community resources, and agencies. Promotes home</p> <p>care services and coordinates care across the continuum.</p> <ul> <li>Maintains up-to-date clinical knowledge of methods, products, trends, research, etc., and </li></ul> <p>communicates this information to staff.</p> <p>Education/Experience Requirements:</p> <ul> <li>Licensed physician, physical therapist, speech language pathologist, occupational therapist, </li></ul> <p>or registered nurse.</p> <ul> <li>Registered Nurse with a Bachelor's Degree (preferred) in Nursing from an accredited program </li></ul> <p>and current license from the State Board of Nursing; or</p> <ul> <li>Therapist with a degree from an accredited program and current license from the </li></ul> <p>corresponding therapy board.</p> <ul> <li>Three (3) years nursing/therapy experience.
