<p>Responsible to direct the functions of clinical nutrition services (such as nutrition assessment, nutrition counseling/consultation, performance improvement), and the leadership of the clinical nutrition team to ensure high quality nutritional care is provided to patients/residents. Ensures patient satisfaction, quality of care, regulatory agency compliance, and good public relations are achieved through the safe and efficient use of resources.</p> <ul> <li>Conducts comprehensive patient screenings by reviewing medical records, including diagnoses, past medical history, and laboratory data, assigning appropriate level of care in accordance with established guidelines </li><li>Communicates patient nutritional needs effectively to CNM, diet technicians, and other dietitians to ensure full awareness, alignment, and continuity of care across the dietetic team. </li><li>Develops accurate and individualized nutritional care plans that reflect each patient's current medical condition and nutritional status. </li><li>Interviews and assesses patients and/or family members to evaluate nutritional status, understanding of dietary requirements, and readiness for dietary changes. </li><li>Monitors and promotes patient satisfaction with meal services and diet education by addressing concerns, reinforcing dietary teaching, and collaborating with the food and nutrition team to support positive patient experiences. </li><li>Provides daily oversight and guidance to diet office staff, in partnership with the Clinical Nutrition Manager (CNM), including kardex review to ensure accuracy of menu selections, diet orders, and proper use of menu "write-ins." </li><li>Completes and documents nutritional assessments in compliance with JCAHO, hospital standards, and Clinical Nutrition Manager expectations, and provides timely and appropriate nutrition recommendations. </li></ul>
