Reporting to the Executive Director, the Clinical Navigator collaborates closely with staff, physicians, and patients to enhance the delivery of patient care services along the continuum of care. As part of a globally-oriented, multicultural team, the Clinical Triage Specialist works closely with team members to assess inbound inquiries for clinical appropriateness and to ensure clinical itineraries are delivered efficiently and expeditiously, while continuously improving the patient's experience. In tandem with the administrative team, he/she assesses patients' clinical journeys to maximize clinical impact with high sensitivity to the special needs of traveling patients.</p> <p>Qualifications</p> <p>Education</p> <ul> <li>Bachelor's Degree Nursing or related field of study required </li></ul> <p>Experience</p> <ul> <li> <p>3-5 years of experience required in Clinical Documentation Improvement (CDI) specialist training, including clinical coding knowledge.</p> </li><li> <p>2-3 years of experience required with chart reviews in electronic medical records, clinical data, supporting or leading work around quality and improvements and creating and/or presenting education material.</p> </li></ul> <p>Knowledge, Skills and Abilities</p> <ul> <li> <p>Review medical records and treatment plans to ensure that they align with established guidelines and standards.</p> </li><li> <p>Participates in department quality improvement initiatives.</p> </li><li> <p>Reviews and applies analytical skills to designated patient population medical record documentation to assess the level of documentation accuracy and completeness.</p> </li><li> <p>Provides clinician specific education regarding quality, outcomes improvement, documentation standards, and actions to improve clinical processes.</p> </li></ul> <p>Roles and Responsibilities:</p> <ul> <li> <p>Elicits patient information from referring MD and outside facility nurse to allow appropriate triage and placement</p> </li><li> <p>Facilitates referring physician callback for specific information</p> </li><li> <p>Provides general information about the program and expertise available for both the medical and surgical services</p> </li><li> <p>Attends patient progression and family meetings as requested</p> </li><li> <p>Troubleshoots potential issues (referring, physician, patient) and collaborates with appropriate stakeholders</p> </li><li> <p>Participates in departmental and organizational committees and programs as appropriate and attends all required meetings, in-services and professional trainings</p> </li><li> <p>Maintains superior interpersonal and communication skills as a member of the healthcare team in order to collaborate effectively with patients, families, staff and community health workers</p> </li><li> <p>Consistently providers high-quality and timely documentation including abstracts, letters of medical necessity and treatment plans.</p> </li><li> <p>Discusses patient data with other physicians and professionals as appropriate in multidisciplinary setting</p> </li><li> <p>Collaborates closely with with intake specialist and patient liaison to assess appropriateness of appointment requests and triage</p> </li><li> <p>Assists in customizing care plans to meet the needs of each patient and ensure their timely implementation</p> </li><li> <p>Regularly reviews with intake specialist and patient liaison the care plans for all ambulatory patients in Boston, to ensure the most appropriate and time-efficient care</p> </li><li> <p>Coordinates as appropriate with physicians and care teams across the system to ensure that all elements critical to the plan and trajectory of care have been communicated both internally and, in tandem with liaisons/client relations ambassadors, to the patient/family and members</p> </li><li> <p>Documents in the medical record as appropriate, as part of the Interdisciplinary team in accordance with the documentation standards</p> </li><li> <p>Utilizes an interpreter when appropriate and incorporates awareness of patient's cultural needs that may impact the plan of care</p> </li><li> <p>Monitors the patient's progress, intervening as necessary and appropriate to ensure that the plan of care and services provided are patient focused and friendly, high quality, efficient, and cost effective</p> </li><li> <p>Collaborates as needed with the interdisciplinary team in the review/revision of the therapeutic plan to expedite and arrange post-acute care</p> </li><li> <p>Assists patient liaison and care providers upon patient departure, with appropriate care hand-off to in-country care teams</p> </li><li> <p>Performs other duties as assigned</p> </li></ul> <p>Additional Job Details (if applicable)</p> <p>Remote Type</p> <p>Hybrid</p> <p>Work Location</p> <p>55 Fruit Street</p> <p>Scheduled Weekly Hours</p> <p>40</p> <p>Employee Type</p> <p>Regular</p> <p>Work Shift</p> <p>Day (United States of America)</p> <p>EEO Statement:</p> <p>Mass General Brigham Incorporated is an Equal Opportunity Employer.
