<p>POSITION SUMMARY:</p> <p>Under the direction of the Health Insurance Access Program Director and Associate Director, plan, direct and coordinate outreach activities designed to raise program awareness and maintain community relationships. Complete all applications for the Local Department of Social Services. These duties include interviewing consumers, documentation collection, data input, enrollment, consumer education and follow-up.</p> <p>RESPONSIBILITIES:</p> <ul> <li>Attend all trainings required by the Local Departments of Social Services, and New York State Department of Health, to acquire, improve, and maintain relevant knowledge and skills. </li><li>Attend additional trainings specific to the Aged, Blind and Disabled population. </li><li>Conduct and participate in outreach events to strengthen rapport with the community and provide education regarding government-sponsored health insurance. </li><li>Travel to partner locations to maintain relationships. </li><li>Provide referrals to other agencies, if applicable. </li><li>Collect and report data to Director and Associate Director. </li><li>Report all issues and concerns to Director and/or Associate Director for resolution. </li><li>Maintain ethical standards including but not limited to HIPAA. </li><li>Assist Disabled, Aged, Blind also known as non-MAGI population by inputting all required household data on application and accompanying documents </li><li>Collect and copy required documents </li><li>Explain submission and approval process </li><li>Provide guidance on the Medicare application process </li><li>Educate consumers regarding changes in circumstances that may affect eligibility as well as reporting requirements </li><li>Assist NON-MAGI population with renewal completion </li><li>Travel to meet consumer, as necessary </li></ul> <p>This is a grant funded position and is contingent upon the availability of continued grant funding and operational needs</p> <p>MINIMUM QUALIFICATIONS:</p> <ul> <li>Ability to handle confidential information in an ethical, professional manner. </li><li>Ability to provide exceptional customer service to all clients. </li><li>Must be a collaborative team-player willing to provide assistance and perform various tasks as needed. </li><li>Must be willing to travel throughout NYC, Rockland and Westchester Counties. </li></ul>
