<p>Position: Project Manager Care Center</p> <p>The Project Manager, reporting to the AVP of the Patient Care Center, will be responsible for the successful development, execution, and ongoing oversight of multiple projects that significantly influence the organization's performance. This role will involve proactively leading projects by establishing clear project goals, creating detailed plans and timelines, identifying key milestones, and managing resources effectively. Additionally, the Project Manager will be tasked with recommending process and product improvements, ensuring both new and existing initiatives align with organizational objectives and drive measurable outcomes.</p> <p>This individual will be responsible for managing communication and ensuring alignment across interdisciplinary teams throughout the project lifecycle. They will track project progress, ensuring that deadlines are met, and provide regular status updates to all impacted stakeholders. This individual will ensure that all project milestones are met in a timely and organized manner.</p> <p>Roles and Responsibilities:</p> <ul> <li>Project Management and Execution: </li><li>Implement a successful project management approach, ensuring repeatable positive outcomes. </li><li>Masterfully manage project scope, schedule, and maintaining focus on task completion. </li><li>Provide regular, timely communication to all project participants regarding project status, challenges, and required actions. </li><li>Process Management and Improvement: </li><li>Lead and manage process changes, ensuring smooth transitions with minimal disruption to operations. </li><li>Lead projects in the following areas: </li><li>Own the engagement playbook and drive its evolution. </li><li>Collaborate with Product Operations and the Patient Experience team on dynamic scripting, engagement enhancement, patient experience evaluations, clinician tool optimization/development, and larger-scale improvements. </li><li>Analyze performance data to identify trends and recommend process improvements to care center leadership. </li><li>Stakeholder Communication and Alignment: </li><li>Ensure clear, ongoing communication between all stakeholders, keeping everyone informed and aligned. </li><li>Align engagement goals with the care center's overall mission and objectives, revisiting these goals regularly to ensure continued alignment. </li><li>Operational Efficiency and Performance: </li><li>Lead initiatives to improve operational efficiencies, including but not limited to: </li><li>Patient visit scheduling. </li><li>Engagement strategies. </li><li>Consistency in operational reporting. </li><li>Monitor and assess Care Center workflows, identifying opportunities for improvement and ensuring adherence to established processes. </li><li>Regulatory Compliance and Risk Management: </li><li>Ensure all processes and care center operations comply with legal and industry regulations, maintaining a focus on quality and compliance. </li></ul> <p>Position Requirements</p> <ul> <li>Bachelor's degree from a four-year college and 5+ years project management experience or any equivalent combination of education and or experience. </li><li>Excellent verbal, written and visual communications skills </li><li>Ability to hold personnel accountable to porrect quality, targets, and timelines </li><li>Ability to multi-task and handle a large volume of concurrent priorities </li><li>Excellent verbal, written, and visual communication skills </li></ul> <p>Preferred Skills/Experience</p> <ul> <li>Relevant work experience in healthcare information technology systems </li><li>Experience with Health Cloud, Sales Force, Microsoft Azure and Athena or similar EMR systems </li><li>Experience working for a health insurance company or client, managed care organization or value-based healthcare provider </li><li>Certification in Project Management (CPM) preferred. </li></ul> <p>Benefits</p> <ul> <li>Opportunity to work in a dynamic, fast-paced and innovative value-based provider organization that is transforming the delivery of kidney care </li><li>Competitive salary and opportunity to participate in the company's bonus program </li><li>Comprehensive medical, dental, vision and life insurance </li><li>Flexible paid leave and vacation policy </li><li>401(k) plan with matching contributions </li></ul> <p>About Monogram Health</p> <p>Monogram Health is a next-generation, value-based chronic condition risk provider serving patients living with chronic kidney and end-stage renal disease and their related metabolic disorders.
