<p>Position Summary</p> <p>The Pharmacist is responsible for accurately monitoring patient medication profiles, filling parenteral and enteral medication orders, providing drug information, and supervising and directing support personnel. The pharmacist will ensure adequate control and documentation of all controlled substances and controlled substance records. In the absence of the Director of Pharmacy, or Pharmacy Technician, the Pharmacist may be required to assume the essential responsibilities and perform their duties.</p> <p>Essential Job Functions/Accountabilities</p> <p>Assesses the appropriateness of therapy and dosing of medications ordered related to population as necessary. Completes clinical informatics tasks, medication reconciliations, home medications reviews, and formulary and order set reviews as necessary.</p> <p>Identifies and specifies pharmacotherapeutic goals for patients.</p> <p>Provides advice on the pharmaceutical and pharmacodynamics properties of drugs including alterations of these parameters in 'special groups' of patients, such as the older adult. Provides clinical consultation and clarification to practitioners.</p> <p>Recommends appropriate, cost-effective therapeutic alternatives to medical staff as needed. Provides education and training to all healthcare professionals including students.</p> <p>Provides education to patients and family members/caregivers regarding medications, how to take or administer them, their effects and side-effects, and how they may affect or improve the patient's disease or medical condition.</p> <p>Provides education to patients on selected diseases or disease processes (e.g. thromboembolic disease and antimicrobial therapy, patient and opioid utilization, glycemic control and diabetes, etc.).</p> <p>Review, evaluate, clarify, and verify medication orders.</p> <p>Suggests changes in dosage strength and/or frequency for medications significantly affected by age, weight, comorbidities, drug interactions or renal clearance.</p> <p>Provides leadership and mentorship to other pharmacists, pharmacy technicians and support staff. Oversees pharmacy support personnel.</p> <p>Reviews policies and procedures relevant to pharmacy practice. Ensures preparation, submission, review, and reconciliation of patient charges. Supports the pharmacy's Joint Commission readiness program.</p> <p>All other duties not specifically assigned.</p> <p>Position Requirements</p> <p>Education: Bachelor of Science in Pharmacy or Doctorate of Pharmacy.</p> <p>Experience: Two (2) to three (3) years hospital experience preferred or new graduate with residency preferred. Three (3) to Five (5) years of demonstrated experience in field or profession preferred.</p> <p>Technical Skills: Prefer Cerner experience. Prefer Informatics experience.</p> <p>Certifications/Licenses/Registrations: Current license to practice as a pharmacist in the state of Ohio; in good standing required. Pharmacist Candidates with active and good standing Ohio Intern license are able to apply if eligible to sit for the NAPLEX and or MPJE within the next 12 months. PharmD candidates will be hired initially as Graduate Pharmacy Intern until required licensing obtained. BLS and ACLS within 1 year of hire.</p> <p>Schedule: Minimum 24 hours per month</p> <p>Status: Per Diem (PRN) Variable</p>
