Job Description Elwyn Adult Behavioral Health Services is hiring for a Licensed Addiction Specialist to provide addictions rehabilitation, individual/group counseling and staff resource education in a therapeutic setting to assist adult individuals with a primary diagnosis of mental illness and co-occurring addiction disorders to achieve social, economic, emotional, and personal independence on their pathways to Recovery. DUTIES AND RESPONSIBILITIES Works as part of the Integrated Team in ensuring the individual's care is coordinated with MA Behavioral Health Community Partners, One Care and DMH Case Managers to facility optimum service delivery. Integrate PRISMModel FOR THE EFFECTIVE TREATMENT OF SERIOUS AND PERSISTENT MENTAL ILLNESS into daily clinical approaches/interventions. Completes Substance Abuse Assessments for all individuals, as indicated. Works collaboratively with Individuals to understand how addictive behavior impacts the Individual in any of the following: financial, social, employment, educational, housing, physical or medical concerns, parenting ability, familial or intimate relationships, engagement with service providers and criminal justice involvement. Develops interventions that address the level of use and potential risk, the current impact of addictive behavior, and the Individuals readiness for change. Facilitates dual diagnosis treatment group on as needed basis, focusing on education/motivation, relapse prevention, and evidence-based practices. Assesses mental health status, safety and any potential medication contraindication. Coordinates substance abuse services and serves as a liaison with other treatment facilities upon discharge. Completes clinical documentation in accordance with FHR Policy/Procedures, Funder Regulations, and CARF Requirements within established timeframes. Clinical documentation clearly reflects individual preferences in assessment process, and goal formulation. Documentation is clearly related to individual-centered goals as specified in the Person-Centered Treatment Plan. Interventions are skill-based. Proactively and preventatively applies principles of non-violent crisis intervention in all situations, to mitigate and/or eliminate escalation of crisis situations. Actively participates in supervision process and team/organizational performance improvement initiatives. Works with the integrated to encourages on-going engagement and involvement of family members in all phases of service delivery and evaluation based on the desires of the individual, this may include ensuring family members understand: (1) the individual's illness; (2) the service delivery model; (3) the roles and functions of the Individual's Team Members and how to contact them; and (4) how the family will be supported. As agreed, upon with individual develops communication plans with family members Notifies supervisor of any pertinent information to ensure quality car EDUCATION/SKILLS/EXPERIENCE REQUIREMENTS: High School diploma or equivalency required Licensed Substance Abuse Counselor (LADC) or equivalent required One or more years of experience in providing support to individuals with serious and persistent mental illness desired Preference will be given to multi-lingual candidates who meet the cultural and linguistic needs of the population served Demonstrated ability to work effectively as part of a team Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities Demonstrated strong attention to detail Demonstrated strong time management and organizational skills Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions Must be able to perform CPR using agency-trained protocols Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record Relevant Post-job Offer Testing/Conditions Knowledge and practice of ELWYN, DPH, and DMH policies, regulations, and standards Must complete ACCS Module Training within 21 days of hire?
