We're not just a healthcare organization; we're a community that thrives on innovation, compassion, and positive change.</p> <p>Position Summary</p> <p>The Pharmacist will manage healthcare of TFHC's high-risk patient population through a collaborative approach to patient care management between the clinical and pharmacy departments.</p> <p>Essential Duties & Functions</p> <ul> <li>Assure safe, effective, and cost-effective use of drugs </li><li>Maintains accuracy in age specific issues </li><li>Strictly follows all HIPAA and accreditation agency guidelines; assures compliance with Tampa Family Health Center policy and procedure with quality related event disclosure(s) </li><li>Maintains currency in Florida Board of Pharmacy laws, rules, and regulations; maintains strict record keeping </li><li>Provide drug information to clinicians and patients </li><li>Strict compliance with formulary management and therapeutic substitution initiatives </li><li>Assure ongoing improvements in therapy and systems through collaborative problem solving, research and participation in facility and department organizational performance initiatives </li><li>Maintain a working knowledge of the 340(b)-drug program </li><li>Direct supervision of Pharmacy Technicians, including training and development as well oversite and monitoring of daily activities, monitoring of refrigerator temperatures and expired meds </li><li>Assist in developing and maintaining quality improvement programs, clinical services, an effective formulary, protocols, guidelines, policy, and procedure </li><li>Participates in multi-disciplinary and pharmacy committees, as deemed appropriate by CPO </li><li>Assist in Adverse Drug Reaction and Medication Error reporting </li><li>Participate in pharmacy accreditation and audit preparation and process </li><li>Responsible for maintaining the operations of pharmacy location, including daily cash and inventory and supplies; ensure workspaces and surrounding areas are clean </li><li>Responsible for maintaining and integrating educational updates on CMS regulations (i.e., fraud, waste, and abuse); currency in developments in the community and pharmaceutical fields </li></ul> <p>Required Education, Certifications, Licenses, & Training (minimum requirements)</p> <ul> <li>Active and unencumbered license to practice pharmacy in the State of Florida </li><li>Current BLS Certification </li><li>Bilingual (English/Spanish) a MUST </li><li>Obtain a Statement of Completion Certificate from 340B UNIVERSITY ONDEMAND during first 2 weeks of employment </li><li>Complete annually - 340B University renewal course, Peer Review as well as Fraud, Waste and Abuse training </li><li>Maintain a valid BLS card (renew every 2 years) </li><li>Certified to immunize required </li><li>Pharm D is a plus </li><li>MTM Experience is a plus </li><li>Residency is a plus </li></ul> <p>Required Knowledge, Skills, and/or Abilities</p> <ul> <li>Ability to work with persons from a wide diversity of social, ethnic, and economic backgrounds </li><li>Proficiency in use of Microsoft Word products, including Word, Excel, and Outlook </li><li>Ability to navigate and enter data into an electronic health record </li></ul>
