Be part of an extraordinary life!</p> <p>You Will Enjoy:</p> <ul> <li>401(k) with company match </li><li>Resident sponsored educational scholarships </li><li>Employee assistance program </li><li>Free electric vehicle charging stations </li><li>Daily Pay - Work today, get paid today </li><li>An exceptional work environment that is both engaging and fun </li></ul> <p>Here are a few of the responsibilities of a Community Social Worker:</p> <ul> <li>Support coordination of resident care and services across the community's continuum of care </li><li>Serve as a liaison between residents, families, physicians, and community staff to ensure clear communication and coordination of care </li><li>Maintain accurate, current, and confidential social service documentation and resident records </li><li>Assist residents in accessing internal and external resources </li><li>Support Social Services in ensuring advanced directives are completed, updated, and properly maintained in resident records </li><li>Support resident wellbeing by assisting with wellness initiatives </li><li>Coordinate short-term and long-term stays within higher levels of care </li></ul> <p>Here are a few of the qualifications we need you to have:</p> <ul> <li>Degree in Social Work, Gerontology, or a related field </li><li>Licensure as a Social Worker in Connecticut is preferred </li><li>2-3 years of experience in long-term care, transitional care, memory care, or equivalent healthcare settings preferred </li><li>Proficiency in technology, including Microsoft Office Suite </li><li>Strong interpersonal and communication skill </li></ul> <p>All new hires must be able to pass our preemployment screening process, which includes a fitness for duty physical, criminal background check, drug test and employment references as required by a licensed residential care facility.
