<p>Position Summary:</p> <p>Provides care to patients acting as an assistant to Physicians in the clinic following established standards and practices. Serves as clinical team lead for nursing related activities in the Outreach Oncology setting.</p> <p>Core Behaviors</p> <p>Dependability, Attendance and Punctuality.</p> <p>a. Promotes customer satisfaction through caring, friendly, professional interactions with patients and families.</p> <p>b. Maintains positive interactions with physicians, nursing staff, and other members of the health care team.</p> <p>Provides direct medical care / treatment to the patient in Hematology/Oncology within the scope of nursing practice, according to established protocols and utilizes evidenced based clinical standards under the supervision of the sponsoring physician.</p> <p>a. Performs comprehensive and accurate health history.</p> <p>b. Documents findings appropriately noting pertinent normal and abnormal findings.</p> <p>c. Establishes medical diagnosis based on assessments, interpretation of patient data, comprehensive review of the patient's history, and diagnostic findings.</p> <p>d. Orders, obtains, and interprets diagnostic tests appropriate for the individual patient according to complaint and physical findings.</p> <p>e. Designs, communicates, and orders appropriate treatment plans of care according to current research and evidence.</p> <p>f. Provides patient/family centered care by communicating with the patient/family regarding the treatment plan and the patient response to care.</p> <p>g. Educates patient/family regarding illness and health promotion.</p> <p>h. Supports Patient and Family centered care by involving the patient and the family in care decisions and adhering to the 8 elements of the model when making treatment decisions.</p> <p>i. Orders medications, therapeutic interventions, nutrition, support services according to protocols and as directed by the collaborating physician.</p> <p>j. Coordinates interdisciplinary care for the patient through consultation with other members of the health care team.</p> <p>k. Arranges admissions and discharges under the supervision of the sponsoring physician I. Institutes appropriate stabilization measures in emergency situations, according to established protocols, research, and best practices.</p> <p>m. Assists Practice Administration in coordination of departmental coverage.</p> <p>Provides leadership and direction for nursing related functions in the Hematology/Oncology outreach setting within the scope of nursing practice, according to established protocols and in collaboration with the Cancer Institute nursing staff.</p> <p>a. Serves as outreach Oncology liaison for nursing with respect to Magnet activities.</p> <p>b. Relays all Nursing communications to Outreach Oncology Nursing staff ensuring that appropriate Physician Office Management staff are included in the communication process.</p> <p>c. Help Establish, in collaboration with Cancer Institute, clinical protocols for the delivery of chemotherapy and other oncologic agents.</p> <p>d. Educates outreach staff Nurses regarding changes to chemotherapy protocols.</p> <p>e. Assists Practice Administration in evaluation of clinical aspects of annual performance appraisals.</p> <p>Efficiency and Effectiveness</p> <p>Interprets, analyzes, and evaluates the patient's response to care.</p> <p>a. Evaluates changes in the patient's condition and responses to prescribed therapies, adjusting plan of care as appropriate.</p> <p>b.
