<p>LPN - Out of District 1:1 Nurse</p> <p>Weymouth Public Schools is a dynamic and inclusive learning community dedicated to "Educational Excellence for Every Weymouth Student." Our highly qualified educators and staff members work to provide personalized attention and support to ensure that every student thrives.</p> <p>This is a part-time position which could lead to a full-time position</p> <p>General Summary:</p> <p>Assist in meeting the physical and healthcare needs of the student with special physical/medical needs during the school day. Promote and maintain the health and well-being of student in the classroom setting in order to enable them to take full advantage of available educational opportunities. Travel to and from the student's home and educational placement, including accompanying the student on specialized transportation as required. This role also has the potential for occasional in-district work.</p> <p>Qualifications:</p> <ul> <li>Valid current license to practice as an LPN in the State of Massachusetts </li><li>Graduate from an accredited practical nursing program </li><li>1-3 years' experience in pediatrics, community health, education, or related field. </li><li>Current CPR certification </li></ul> <p>Essential Duties:</p> <p>To the extent of his/her generic and continuing education and experience, the licensed practical nurse shall assume responsibility for the following health services, under the direction of the school nurse and Director of Health Services. Will require travel to support students placed in out-of-district programs.</p> <p>Nursing Responsibilities:</p> <ul> <li>Travel to and from the student's home and educational placement, including accompanying the student on specialized transportation as required. </li><li>Collaborate with the school nurse/lead nurse and the parents to develop and implement an individual health care plan for the assigned student. </li><li>Maintain an accurate health record for the student. Document health-related activities and treatments on daily logs and/or a computer-based health record program as appropriate. </li><li>Document administration of medications, treatments, first aid, and illness care provided to students-complete accident reports. </li><li>Communicate with parents as needed regarding the student's health status. </li><li>Administer medications and perform treatments as ordered by the primary physician in the classroom setting. </li><li>Provide emergency care for injuries or sudden illness, then refer to the primary care physician or the appropriate medical facility. </li><li>Collaborate with the school nurse to screen all new students in the classroom to ensure compliance with immunization law. </li><li>Collaborate with the school nurse to recommend exclusion and re-admission monitoring for students with infectious or communicable diseases. </li><li>Maintain medical equipment and supplies as appropriate in the classroom per student's Individualized Healthcare Plan (IHCP) </li><li>Collaborate/cooperate with community providers and state agencies to meet the health and social service needs of student. </li><li>Be a resource to students, parents, principals, and teachers regarding health matters. </li><li>Identify health and safety hazards in the classroom and advise the school Nurse, Director of School Nursing, and teacher(s). </li><li>Demonstrate knowledge of, and the ability to administer medications appropriately, in keeping with school policy on the administration of medication in school. </li><li>Other duties as assigned by the School Nurse, Nurse leader, per student's IHCP. </li><li>Must be experienced with tracheostomies and tube feedings </li><li>Experience preferred with seizures and AFO's </li><li>Provide personal care, assistance, and supervision, as needed, for 1:1 students, including: </li><li>Dressing and undressing </li><li>Toileting, including diaper change </li><li>Grooming </li><li>Feeding </li><li>Positioning </li><li>Transfers: i.e., may include heavy lifting </li><li>Hygiene </li></ul> <p>Educational Responsibilities:</p> <ul> <li>Assist student with proper body mechanics related to academic work </li><li>Assist student in using adaptive equipment </li><li>Assist with maintenance of classroom supplies, including cleaning and care of equipment. </li></ul> <p>EQUAL OPPORTUNITY EMPLOYER</p> <p>Weymouth Public Schools is committed to maintaining a work and learning environment free from discrimination on the basis of race, color, religion, national origin, pregnancy, gender, sexual orientation, marital/civil union status, ancestry, place of birth, age, citizenship status, veteran status, political affiliation, or disability, as defined and required by state and federal laws.</p>
