<p>Job Requirements</p> <p>Position Summary</p> <p>Facilitate patient care through review and processing of the patient's medication orders and the timely distribution of medications. Communicate with other healthcare personnel to optimize the medication management of a patient. Formulate system process improvements to optimize patient care in the area of expertise. Coordinate and evaluate systemic process improvements in the medication management process.</p> <p>Minimum Requirements</p> <p>Education</p> <ul> <li>PGY2 in area of specialty or </li><li>PGY1 +BCPS + 5 years clinical experience in a specific patient population* or </li><li>PGY2 + BCPS<em> (</em>specialty certification required if available) </li></ul> <p>Experience</p> <ul> <li>Experience in health system pharmacy including experience with computer systems and their applications to the clinical and operational aspects of pharmaceutical care. </li></ul> <p>License/Registration/Certifications</p> <ul> <li>SC Pharmacy License - Will work under direct supervision of licensed pharmacist until license obtained. </li></ul> <p>Preferred Requirements</p> <p>Preferred Education</p> <ul> <li>N/A </li></ul> <p>Preferred Experience</p> <ul> <li>N/A </li></ul> <p>Preferred License/Registration/Certifications</p> <ul> <li>N/A </li></ul> <p>Core Job Responsibilities</p> <ul> <li>Compounds, prepares, labels, and dispenses medications (sterile and non-sterile) for all age groups according to accepted standards of practice as assigned. </li><li>Interprets medication orders, monitors drug regimens, and performs interventions when order clarification is necessary or when therapeutic problems are identified. </li><li>Works with other health care providers, including participating in rounds and other patient care activities, to optimize medication regimens using evidence-based medicine and standards of care. </li><li>Expand and/or optimize departmental services </li><li>Precept APPE Advanced Institutional students. </li><li>Oversee residency application process and program; Precept PGY1 residents as assigned. </li><li>Designs and performs quality improvement projects. Demonstrates the ability to implement new programs/improvements with minimal supervisory assistance and develop metrics to assess the impact of changes made </li><li>Supervises support personnel in a consistent manner. </li><li>Customer satisfaction </li></ul>
