St. Vincent's is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network and is the system's second-largest hospital.</p> <p>We are seeking a Transitions of Care Pharmacist to join our inpatient pharmacy team and play a key role in ensuring safe medication management for patients as they move through the continuum of care.</p> <p>In this role, you will partner closely with physicians, nurses, case management, and outpatient pharmacy to optimize medication therapy and provide patient-centered counseling that supports adherence and reduces hospital readmissions. This position is ideal for a pharmacist who is passionate about improving patient outcomes through direct patient counseling and interdisciplinary collaboration.
