<p>The VA Northeast Ohio Healthcare System is recruiting for a Community Living Center Social Worker located at the Wade Park campus. The incumbent will provide services as part of an interdisciplinary team for Veterans receiving skilled and/or short term nursing home care; providing high quality case management, clinical treatment, advocacy, and coordination with appropriate VA and community service providers/agencies as needed by the Veteran.</p> <p>The Social Worker duties include but are not limited to:</p> <ul> <li>Conduct psychosocial assessments of the Veteran, family members and significant others identifying the Veteran's problems, strengths, coping skills and weaknesses. Demonstrates an understanding of the range of treatment and skills needed to work with adult patients (including geriatric patients) related to their physical limitations, psychological needs and age associated illnesses and conditions. </li><li>Carries a caseload of CLC patients and is responsible for identifying the resource and referral needs of patients, develop discharge plan for patients based on these factors. </li><li>Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. </li><li>Educates Veterans and/or caregivers of the available services and assisting them in establishing the appropriate referrals based on the Veteran's preference or that of their surrogate decision-maker. </li><li>Works with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills. </li><li>Maintains general knowledge of veterans' benefits and services relating to special programs (e.g. Caregiver Support), service-connected compensation, non-service-connected pension.
