Adherence to post discharge plan(s) of care and the holistic health and functioning of the patient/family. Serves as a member of the interdisciplinary health team. Collaborates with departmental/organizational support services. Assists in the development, maintenance and evaluation of the Prisma Health Medical Management Social Work Services and performance improvements.</p> <p>Essential Functions</p> <ul> <li> <p>All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Coordination and collaboration with ancillary services, providers, and agencies.</p> </li><li> <p>Implementation/coordination of identified discharge planning/placement needs. Works closely with others in an effort to optimize quality and minimize cost.</p> </li><li> <p>Provides post-discharge follow-up when deemed appropriate.</p> </li><li> <p>Assists in development/maintenance and evaluation of clinical data.
