<p>Pay Range:</p> <p>$45.84 - $68.76</p> <p>A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.</p> <p>Hospital Sisters Health System (HSHS) is seeking a Manager-Care Management to join our team. Implements an organized, hospital-wide care management program. Promotes an interdisciplinary process that evaluates a patient's progress from admission through discharge. Ensures that patient's care and plans for discharge are provided in the appropriate setting and in a timely, cost-effective manner. Variations from expected outcomes are analyzed and information shared to further improve plan for discharge, utilization or resources, and quality of patient care.</p> <p>Position Specifics:</p> <p>o Department: Care Management</p> <p>o Core Function: Leadership</p> <p>o Schedule: Full Time</p> <p>o Facility: St. Vincent's Hospital, travel throughout WI market, primarily on-site with opportunity for occasional work from home</p> <p>o Location: Green Bay, WI and WI market</p> <p>o Compensation that aligns with your experience</p> <p>Education Qualifications</p> <ul> <li>Bachelor's degree in Nursing or Social Work is required. </li><li>Master's degree in Nursing or Social Work is preferred. </li><li>In lieu of bachelor's degree, diploma or an associate's degree in nursing or in relevant clinical field with a minimum of 3 years of case management experience may be considered. </li></ul> <p>Experience Qualifications</p> <ul> <li>3 years leadership experience in a progressively responsible role is required. </li><li>2 years of care management experience is required. </li></ul> <p>Certifications, Licenses and Registrations</p> <ul> <li>Licensed Registered Nurse (RN) or Licensed Social Worker (LSW) in the state of practice is required. </li><li>In lieu of licensure in Nursing or Social Work, may consider candidates with other clinical credentials. </li><li>Case Management certification (American Case Management Association (ACM), Commission For Case Manager Certification (CCM), Registered Nurse Board Certified (RN-BC) is preferred. </li></ul> <p>Scheduled Weekly Hours:</p> <p>40</p> <p>Throughout communities in Illinois and Wisconsin, 13 hospitals, numerous community-based health centers and clinics, our 13,000+ colleagues have built a culture based on our solid core values of respect, care, competence, and joy.
