They are dedicated to patient and family centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social and resource issues that impact individual pediatric patients, families and at-risk populations. Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.</p> <p>Essential Functions</p> <p>Provide supervision to interns and LCSW's</p> <p>Provides psychosocial assessments of pediatric patients and families with social, emotional, interpersonal and/or environmental issues. Formulates biopsychosocial assessment, disposition and treatment plans.</p> <p>Provides mandated assessments when abuse is suspected in a child and specialized safety assessment when violence is suspected</p> <p>Participates in patient care conferences.</p> <p>Reviews and modifies treatment plans as needed, collaborates with other social workers as needed and serves as a resource.</p> <p>Participates in discharge planning.</p> <p>Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.</p> <p>Strong Critical thinking skills</p> <p>Prepares and maintains all pertinent records, statistics, and progress notes.</p> <p>Required for All Jobs</p> <p>Performs other duties as assigned</p> <p>Complies with all policies and standards</p> <p>Does this position require Patient Care (indirect/direct)? Yes</p> <p>Qualifications</p> <p>Education</p> <p>Master's Degree Mental Health & Behavioral Medicine required or</p> <p>Master's Degree Social Work required</p> <p>Can this role accept experience in lieu of a degree?
