<p>Bilingual Service Coordinator/Case Manager - Early Intervention (Children Birth-3)</p> <p>Developmental Disabilities Resource Center of Lakewood, CO is seeking to hire a full-time Bilingual Service Coordinator/Case Manager - Early Intervention (Children Birth-3) to provide resource coordination to children (birth-3). Work will primarily be remote with some travel around Jefferson County.</p> <p>COMPENSATION</p> <ul> <li>$25.25-$27.78/hour DOQ. </li><li>36 hours per week/4 days per week, with Friday's off. </li><li>Excellent benefits including full medical, dental, vision, 401k, paid vacation, paid holidays, and paid sick time. </li><li>Eligibility to participate in the Public Student Loan Forgiveness (PSFL) Program </li></ul> <p>A DAY IN THE LIFE AS A Bilingual SERVICE COORDINATOR/CASE MANAGER - Early Intervention (Children Birth-3)</p> <p>As a Service Coordinator/Case Manager - Early Intervention (Children Birth-3), you are responsible for the planning, coordinating, and monitoring of Early Intervention (EI) services provided to children with developmental delays and their families. For children eligible for EI services, this position facilitates the Family Assessment and the Individualized Family Service Plan (IFSP) process with optimal participation of the family and extended IFSP team. The Service Coordinator serves as the primary contact for families from referral to the child's exit from early intervention services at age three (3). This position also informs families of their rights and procedural safeguards and how to exercise them.</p> <p>QUALIFICATIONS FOR A SERVICE COORDINATOR/CASE MANAGER - Early Intervention (Children Birth-3)</p> <ul> <li>Bachelor's degree in Early Childhood Education, Social Work or related degree in Human Services preferred. </li><li>Minimum 5 years of experience in case management or appropriate combination of education and experience required for applicants without a degree. </li><li>Case Management and experience working within the field of Intellectual and Developmental Disabilities, Early Childhood or related systems strongly preferred. </li><li>Fluent in Spanish and English.
