<p>The Therapist is a valuable member of our Behavioral Health Outpatient Program who provides evaluation, psycho-social education, mentoring, and treatment services to clients and assists clients in developing and accomplishing treatment goals in individual and group settings. Provides consultation and education to community groups and emergency services.</p> <p>To be successful in this role, we are looking for someone who</p> <ul> <li>Participation in councils and teams, and making suggestions to improve a process. </li><li>May provide clinical consultation to other staff as agreed upon with the Program Coordinator. </li><li>Provides community education services as assigned and as self-initiated. </li><li>May assist in the facilitation of client transportation. </li></ul> <p>Schedule: 37.5 hours a week</p> <ul> <li>Monday 8:30 am - 4:30 pm </li><li>Tuesday 8:30 am - 8:00 pm </li><li>Wednesday 8:30 am - 5:00 pm </li><li>Thursday - 8:30 am - 8:00 pm </li><li>Friday - Off </li><li>Conduct crisis intervention/evaluation/referral for persons present at the program. </li><li>Performs initial evaluations and assessments of potential clients regarding their appropriateness for participation in the program. </li><li>Provides direct services to clients including individual and group treatment. </li><li>Focuses on client empowerment in providing treatment, seeking to assist clients to build strengths to meet their treatment needs. </li><li>Facilitates with clients and collaborates with the treatment team to develop, review, and update treatment goals and referrals internally and externally. </li><li>Provide education and direction to clients, family members, and/or significant others. </li><li>Organizes and participates in treatment-related clinical conferences and staffings on assigned clients. </li><li>Takes responsibility for training in and documenting clinical chart requirements to meet program and state standards. </li><li>Provides emergency evaluations of clients regarding their need for involuntary hospitalization, if meet the criteria as a qualified examiner. </li><li>Serves as an LPHA and/or QMHP, which includes reviewing and signing clinical assessments, individual treatment plants, and other appropriate clinical documentation. </li><li>Coordinates weekly clinical staffings and provides clinical consultation as directed by the Program Coordinator. </li><li>Master's degree in a related field, and </li><li>Successful completion of 1,000 practicum hours, or </li><li>One (1) year of documented clinical experience under the supervision of a QMHP. </li></ul> <p>As the largest human service provider in Lake County, we believe that services must be available without barriers.
