<p>Harbor is seeking a Job Coach to join the Toledo team! The Job Coach position provides vocational rehabilitation services as well as job coaching services to Jobs and Family Services referrals.</p> <p>Position is full-time, 1st shift.</p> <p>Education/Experience/Other Requirements:</p> <ul> <li>Two years teaching/training experience required. Bachelor's degree with teaching/training experience preferred. </li><li>Must be proficient and accurate in computer use, including Microsoft Word. </li><li>Must have a valid driver's license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor's vehicle insurance carrier. </li><li>CPR/First Aid Certification required within 90 days of employment. </li></ul> <p>Essential Job Competencies/Primary Duties:</p> <p>DOCUMENTATION</p> <ul> <li>Responsible for the quality and completeness of the electronic record. </li><li>Responsible for accurate and timely documentation which includes, but is not limited to: progress notes, sheets, ROAR's, Monthly Participant Evaluations, Corrective Action Plans, job coaching plans, iWAM alerts, Incident Report forms and any other forms necessary to document coaching services. </li><li>Types documentation in the format appropriate and acceptable to Harbor. </li><li>Maintains active communication with funding source and supervisor regarding client progress. </li></ul> <p>EVALUATION</p> <ul> <li>Provides Job Coaching, Job Seeking Skills Training (JSST), and/or job development activities for Opportunities for Ohioans with Disabilities and/or Lucas County Job and Family Services referrals. </li><li>Evaluates work site for appropriateness for participant placement (e.g., safety, supervision availability, enough work to keep participants busy, etc.). </li><li>Ongoing evaluation of WEP site and/or worksite to ensure that business is continuing to meet its responsibility and program requirements. </li><li>Analyzes job duties and work environments at WEP sites and worksites; breaks down jobs into trainable steps for client. </li><li>Evaluates worksite/employee job match and may complete worksite job analysis. </li><li>Assesses client readiness for employment and formulates appropriate interventions to assist the client in making progress towards setting and achieving their employment goals. </li><li>Assists the client in identifying his/her interests, skills, and aptitudes to assist in exploring possible vocational choices. </li><li>May assists the client in gaining information on the effects of employment on entitlements / financial assistance that the client may be receiving (i.e., SSI/SSDI, Medicaid, Medicare, Section 8, Food Stamps) to assist the client in making an informed choice. </li></ul> <p>ONGOING INTERVENTIONS</p> <ul> <li>Provides individual and/or group job coaching at WEP or work site. </li><li>Assists the client in developing employment opportunities as well as the skills to select, obtain, and maintain jobs. </li><li>Makes home visits/outreach to engage clients in vocational services as needed </li><li>Consults with supervisor regarding appropriate referrals for client to vocational or community programs/services </li><li>Provides support, education (i.e., benefits counseling, risks and benefits of employment, and job seeking skills) and consultation to clients, families, and/or significant others. </li><li>Actively develops employment opportunities and assists clients in learning job skills as needed, individually or in groups. </li><li>Performs community outreach for the purpose of client engagement, and necessary support services. </li><li>Provides coaching and mentoring assistance as needed for the purpose of maintaining and retaining employment. </li><li>Provides support to participants to assist them in problem solving and managing barriers to client participation and/or employment (e.g., linking with childcare resources, health care resources, transportation, etc.). </li><li>Keeps current with trends and developments related to essential job competencies and demonstrates continued growth. </li></ul> <p>COLLABORATION</p> <ul> <li>Assists the client and family in increasing social support skills and networks. </li><li>Coordinates evaluations and assessments by internal and/or external providers. </li><li>Monitors all services identified in the referral, IEP, BESI and DSI. </li><li>Provides assistance in gaining access to essential community resources and linking with appropriate vocational resources (OOD, ODJFS, BWC, etc.). </li></ul> <p>About Harbor:</p> <ul> <li>A leading provider of mental health and substance use treatment for over 100 years </li><li>350+ clinical staff serve over 24,000 clients across multiple locations and in the community each year </li><li>Services ranging from counseling, pharmacological management, primary care, psychological testing, case management, substance use treatment, residential services, vocational program, and more! </li></ul> <p>Why Work for Harbor?</p> <p>It is fast-paced and challenging, but you will have a lot of fun in the process.
