In this role, you will evaluate the psychosocial and mental health needs of patients to support transition planning, crisis intervention, and coordination of both internal and external services. You will assess and implement intervention strategies related to behavioral health, addiction, end-of-life care, and informed decision-making regarding therapeutic options. Our Clinical Social Workers serve as advocates not only for patients, but also for their families, caregivers, surrogates, and healthcare teams, helping ensure compassionate, comprehensive care throughout the patient journey.</p> <p>We will consider candidates with their Clinical Social Work Associate license working towards a Licensed Clinical Social Worker license for a Clinical Social Worker I position ($40.06 min - $57.28 max) or candidates with their Licensed Clinical Social Worker license for a Clinical Social Worker II position ($42.06 min - $60.14 max).</p> <p>Legacy Emanuel Medical Center in North Portland plays a vital role as a local and regional leader in serious clinical illness or injury. With around-the-clock expertise for critical health issues, including experts in trauma, heart care, burns, significant wounds, stroke, brain surgery and more, Legacy Emanuel is central to the health of our community and critical to the care of the Northwest.</p> <p>Responsibilities</p> <ul> <li>Provides social work services in a variety of adult and pediatric settings across the health care continuum with a focus on holistic patient-centered care and attention to the needs of vulnerable populations. </li><li>Works both independently and collaboratively as part of an interdisciplinary team. </li><li>Key services include (but are not limited to) comprehensive evaluation of psychosocial and mental/behavioral health status, legal and safety considerations, substance use or addiction, chronic illness, or end of life issues with the use of brief counseling, transition planning, crisis management and planning and coordination with internal and community stakeholders. </li><li>Social work intervention may be with individuals, family members or groups. </li><li>Throughout the care continuum, social workers will utilize a strengths-based approach with the aim to enhance the capacity of people to address their own needs. </li></ul> <p>Qualifications</p> <p>We offer two levels of Social Worker positions based on qualifications:</p> <p>Clinical Social Worker I (CSWA):</p> <ul> <li>Education: Master of Social Work degree from an accredited School of Social Work required. </li><li>Experience: Prefer 1 year of relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization. </li></ul> <p>Clinical Social Worker II (LCSW):</p> <ul> <li>Education: Master of Social Work degree from an accredited School of Social Work required. </li><li>Experience: 2 years relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization. Preferred: 3 years' relevant experience. </li></ul> <p>Skills:</p> <ul> <li>Broad knowledge of the principles of social work and the ability to apply them with individuals, families and/or groups integrating knowledge of normal development and the aging process, changes related to illness, and the needs of patients with acute, chronic, and terminal illness in assessment and intervention with attention to cultural and literacy implications. </li><li>Knowledge of medical, mental health and substance use diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology </li><li>Strong organizational, communication and collaboration skills, including an ability to effectively triage patient and family needs, function as member of an interdisciplinary team, identification and collaboration with community stakeholders or resources and written documentation. </li><li>Knowledge about social determinants of health, principles of diversity/equity/inclusion and culturally responsive practice with populations who have experience historical marginalization </li><li>Demonstrates ability to locate, utilize and advocate for patient/family access to hospital/ community resources </li><li>Exhibits knowledge of legal/regulatory issues and an ability to be a resource to all team members regarding such issues </li><li>Ability to navigate electronic medical records and effectively utilize basic computer programs. </li><li>Knowledge of legal, regulatory and compliance requirements and an ability to be a resource to all team members regarding such issues and knowledge of current health care environment. </li></ul> <p>Licensure</p> <p>Clinical Social Worker I (CSWA):</p> <ul> <li> <p>Clinical Social Work Associate (CSWA, working toward LCSW) required in OR.</p> </li><li> <p>Licensed Social Worker (LSWAIC, working toward LICSW) required in WA.</p> </li><li> <p>Associates in WA may begin work with a "pending" associate license and may work for a maximum of 120 days while awaiting licensure approval. If approval is not obtained within 120 days (or license denial is issued) the associate would be required to cease work immediately.</p> </li><li> <p>American Heart Association (AHA) issued BLS Provider certification required.</p> </li></ul> <p>Clinical Social Worker II (LCSW):</p> <ul> <li>Licensed Clinical Social Worker (LCSW) required in OR. </li><li>Licensed Independent Clinical Social Worker (LICSW) required in WA. </li><li>American Heart Association (AHA) issued BLS Provider certification required. </li></ul> <p>Pay Range</p> <p>USD $42.06 - USD $60.14 /Hr.</p> <p>Our Commitment to Health and Equal Opportunity</p> <p>Our Legacy is good for health for Our People, Our Patients, Our Communities, Our World.
