<p>Under the direction and supervision of a physician, the Nurse Practitioner provides professional medical services for AACI Health Center patients.</p> <p>Duties and responsibilities</p> <ul> <li>Provide assessment, diagnosis and treatment </li><li>Prescribe and dispense medications </li><li>Develop and implement treatment plans with patients and other care team members </li><li>Check vitals and conduct physical and reviews patient case histories </li><li>Provide continuing, comprehensive healthcare maintenance and medical care to patients, including preventive care, and behavioral and community health care </li><li>Educate patients on topics such as disease process, medication, prevention, and compliance </li><li>Consult with and referral to affiliated subspecialists </li><li>Work as part of an integrated care team </li><li>Collaborate with behavioral health and wellness colleagues </li><li>Participate in on-call after hours rotation </li><li>Document and maintain clinical records per appropriate standards and regulations </li><li>Participate in team meetings </li><li>Participate in clinical quality improvement processes including peer reviews, data collection, health outcomes reporting, clinical audits, program evaluation </li><li>Provide coverage for other clinicians when needed </li><li>Complete other duties and related projects as assigned </li></ul>
