<p>Employment Type:</p> <p>Full time</p> <p>Shift:</p> <p>Day Shift</p> <p>Description:</p> <p>Social Worker (MSW)</p> <p>Shift: Mon to Fri, 8 am to 430 pm, no nights/weekends/holidays</p> <p>Status: Full time</p> <p>Location: LIFE at St. Francis Riverfront, Wilmington, DE</p> <p>Position Purpose:</p> <p>This position will plan, organize, and implement social services for PACE Organization (PO) participants and families including case management and participant advocacy. Responsible for but not limited to assessment, treatment, teaching and counseling participants, caregiver, or other appropriate representatives.</p> <p>Position Details:</p> <p>May require local occasional local travel.</p> <p>Most of the work will be done in the facility.
