<p>Only applicants who meet the Minimum Qualification Requirements and Selective Requirements (listed below) will be placed on the eligible list. </p> <p>The Iowa Department of Health and Human Services (HHS), Division of Compliance, Bureau of Data Privacy and Strategy is seeking a Hometown Connections Coordinator (Executive Officer 2) to join our team! </p> <p>The Iowa Department of Health and Human Services (HHS) is seeking a skilled and detail-oriented individual to serve as the Hometown Connections Coordinator for the Centers for Medicare & Medicaid Services' Rural Health Transformation Program, opportunity #: CMS-RHT-26-001, also known as Iowa's Healthy Hometowns. This position plays a critical role in coordinating all aspects of the Hometown Connections, Health Hubs initiatives, including development of work plans, goals, and strategies for technical assistance providers and pilot projects to meet federal funding requirements and support HHS partner needs. </p> <p>This position is housed within the Data Privacy and Strategy sub-division of the Compliance and Administration Division at Iowa HHS. This is an exciting opportunity to contribute to systems-level change and capacity building across Iowa's health and human services landscape. </p> <p>Responsibilities: </p> <p>This position will serve as the Hometown Connections Coordinator for the Centers for Medicare & Medicaid Services' Rural Health Transformation Program, opportunity #: CMS-RHT-26-001, also known as Iowa's Healthy Hometowns. The Coordinator will serve as the lead for the initiative and will coordinate all aspects of the program with HHS leadership, staff and key rural health partners. Experience with contract management, hub and spoke model systems development, rural healthcare initiatives, and health care provider engagement work is highly recommended. </p> <p>What You'll Do: </p> <ul> <li>Coordinate all aspects of the Hometown Connections, Health Hubs initiative, including development of work plans, goals, and strategies for technical assistance providers and pilot projects to meet federal funding requirements and support HHS partner needs. </li><li>Manage contracts and budgets for the initiative, ensuring deliverables are met, appropriate funding is allocated, and required federal reports (e.g., FFATA, subrecipient monitoring) are completed. </li><li>Collect and analyze performance data from contractors to evaluate progress, identify areas for improvement, and collaborate with Healthy Hometowns team members to support program implementation. </li><li>Provides direction and coordinates with the Healthy Hometowns Economist, Health Hubs and Combat Cancer technical assistance providers, and pilot project sites to ensure evidence based hub and spoke programming and transformative rural healthcare models are integrated and implemented appropriately within the initiative. </li><li>Analyze laws, regulations, and best practices to recommend hub and spoke care model strategies and operational improvements that enhance provider communication, patient outcomes, and service delivery. </li><li>Lead partnership development by engaging healthcare systems, community-based organizations, and HHS contractors to expand pilot project opportunities and strengthen program impact. </li><li>Prepare and deliver presentations to HHS leadership and external stakeholders, facilitates meetings, and participates in committees and workgroups to promote program development and share initiative outcomes. </li></ul> <p>Benefits</p> <p>Working for the State of Iowa comes with its perks. For more information, please visitwww.e-verify.gov.</p> <p>246 Grant Writing and Management</p> <p>6 months' professional level experience as one of a team or as the sole organizational resource managing the grant program, which includes all of the following: writing grant applications/proposals, managing grant funds, providing technical assistance to sub-grantees, monitoring grants, and grant performance reporting. Experience must have been a major or dominant function of the job. May also include experience reviewing, approving/awarding, and monitoring others' grant proposals, but this alone does not qualify.</p> <p>Applicants must meet at least one of the following minimum requirements to qualify for positions in this job classification: </p> <p>1) Graduation from an accredited four-year college or university with a degree in any field, and experience equal to four years of full-time professional-level work in program administration, program development, program operations, or management. </p> <p>2) A total of eight years of education and/or full-time experience (as described in number one), where thirty semester hours of accredited college or university course work in any field equals one year of full-time experience. </p> <p>3) All of the following (a, b, and c): a. Two years of full-time professional-level work experience in program administration, program development, program operations, or management; and b. A total of four years of education and full-time experience (as described in part a), where thirty semester hours of accredited college or university course work in any field equals one year of full-time experience; and c. A total of two years of graduate-level education and full-time experience (as described in part a), where twenty-four semester hours of accredited graduate college or university course work in a public-service-related area (e.g., public or business administration, social work, public health, law, education, engineering) equals one year of full-time experience.
