<p>Job#: 3022264</p> <p>Job Description:</p> <p>Position Title: IDD Care Manager</p> <p>Location: Remote in NC</p> <p>Duration: 6-month W2 Contract. Conversion possible</p> <p>Intellectual/Developmental Disability (I/DD) Care Manager-1915i is responsible for providing complex care coordination and monitoring to members having a primary Intellectual or Developmental Disability, which may include a secondary mental health or substance abuse, and/or complex medical/physical health need. This position requires a dynamic, proactive approach to assessment, monitoring and complex coordination of care, to ensure quality support and consistent adherence to various waiver, Medicaid, and State Funded requirements.</p> <p>IDD/Care Manager Assists with integrated care management through multi- disciplinary collaborations and referrals, including but not limited to: assistance with the TCM opt in/opt out process, completion of care needs screenings, 90 day transition plans, completion/updates of CMCAs and care plan development, and service coordination as needed/deemed appropriate. Coordinates with internal and external specialty teams, including regional care teams, foster care teams, other community agencies and programs as needed. This position has a primary focus on members that have Intellectual Developmental Disability, serving both children, adolescents and adults.</p> <p>I. DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES: Essential Functions describe the job duties that must be performed to achieve the important end results for which the job exists.</p> <p>The I/DD Care Manager-1915i is responsible for (though not limited to) the following: Assessment and Planning:</p> <ol> <li> <p>Utilizes person centered planning methods/strategies to gather information, to get to know the members supported and to adequately address specific needs of the individual.</p> </li><li> <p>Provides education and support to members in learning about and exercising rights and responsibilities.</p> </li><li> <p>Scheduling and completion of NC Medicaid 1915i Assessment.</p> </li><li> <p>Consistently completes Discovery activities (information gathering and assessment) as a component of the planning process.</p> </li><li> <p>Ensures that members/legally responsible persons are informed of services available (all types of Medicaid and state-funded services), service options available (e.g. Individual/Family Direction for Innovations participants), processes (e.g. requirements for specific service), Natural and community services, and so forth.</p> </li><li> <p>Assists members/legally responsible persons in choosing service providers, ensuring objectivity in the process.</p> </li><li> <p>Assists the member supported to direct the planning process/plan development, to the extent desired by the member.</p> </li><li> <p>Facilitates timely development of the Comprehensive Care management Assessment, Individual Support Plan/Care Plan, Crisis Plan and other assessments as needed.</p> </li><li> <p>Updates the member's Individual Support Plan/Care Plan to include 1915i services and supports.</p> </li><li> <p>Ensures corrections are made timely as requested or needed from State wide</p> </li></ol> <p>vendor's (Carelon) review of completed assessments.</p> <ol start="11"> <li> <p>Adheres and follows step-actions, or workflows provided to complete 1915i assessments.</p> </li><li> <p>Ensures annual re-assessments occurs for members based on caseloads or</p> </li></ol> <p>updated if member's circumstances changes.</p> <ol start="13"> <li> <p>Actively collaborates with members supported and members of the support team to ensure development of a comprehensive plan that is reflects the member's needs and desired life goals as well as, the physical, behavioral health and social determinant needs, service delivery and health and safety of the member.</p> </li><li> <p>Collaborates with integrated care team members, including but not limited to Specialty Care team and Neighborhood Connections Department to provide integrated care.</p> </li><li> <p>Assists members in navigating the healthcare and social support systems to ensure overall health and wellbeing.</p> </li><li> <p>Promotes use of natural/community resources through the assessment, planning, and linkage process.
