<p>Job Summary</p> <p>Serve as a consistent care coordinator ensuring timely access and seamless coordination of services throughout the continuum of care. The nurse navigator utilizes a focused approach in following and directing patients. In the process of following and directing those patients the navigator will educate the patient/family/caregiver regarding the disease process and the necessity of close, continual monitoring. The Nurse Navigator will possess the attributes of a leader/self-starter/team player enabling them to serve as a bridge between the professional staff and the patient/family/caregiver. The Nurse Navigator will possess exceptional communication skills, both written and oral that will allow them to build collaborative relationships to enable the patient/family/caregiver to better navigate the health care system. The Nurse Navigator will work closely with the Oncology/Lung Nodule Navigator and STS Database to ensure care is maintained.</p> <p>Responsibilities</p> <ul> <li>Assumes leadership for the CAMC Thoracic/Esophageal Programs. As such, drives execution by translating strategic priorities in operational reality. Drives improvement by leading through vision and values for the thoracic patient population. Demonstrates the ability to lead. </li><li>Cost effectively manages patients across the entire continuum of care. </li><li>Utilizes care management practice and skills to effectively manage clinical, quality, and economic outcomes for the patient population. </li><li>Develops, implements, and evaluates clinical practice guidelines/care paths for the patient. </li><li>Coordinates patient care across all settings with all healthcare providers. Interface with attending and consulting physicians, residents, staff nurses, and other members of the multidisciplinary team to identify and address system processes, which impact the timely facilitation and delivery of care. Address these issues to provide optimal care </li><li>Provides educational information and resources to patients in order to assist patient and family with coping, foster self-care activities and assure high quality value-added. </li><li>Assists in coordinating resource referrals to optimize patient outcomes and create cost effective opportunities. </li><li>Collaborates with health team regarding community educational needs and the delivery of value added services. </li><li>Collaborate with internal and external resources in development and maintenance of a telemedicine and or remote monitoring network. </li><li>Evaluates clinical, quality, and economic outcomes of patient care for the population. </li><li>Collects and evaluates data related to patient population in order to provide information for ongoing quality improvement. </li><li>Monitors clinical outcomes and system issues related to quality care delivery, developing quality filters, audits, and case reviews, identifying trends and helping to outline remedial actions while maintaining confidentiality. Responsible for the implementation, review, and revision of standards, policies, procedures, follow ups, and protocols in collaboration with the Oncology/lung nodule navigator and STS Database. </li><li>Maintains an active role in problem identification and problem solving based on current research and professional practice standards. </li><li>Actively participates in committees related to care, prevention, and screening. Tumor Board Meetings. </li><li>Establishes and maintains knowledge base of on-going clinical research activities. </li><li>Assumes a leadership role in the development of standards for patient education. </li><li>Develop, coordinate, and provide community outreach activities for prevention, education and health screenings of participants. Includes development of education for CAMC patients and identification of Performance Improvement issues. </li></ul> <p>Knowledge, Skills & Abilities</p> <p>Patient Group Knowledge (Only applies to positions with direct patient contact)</p> <p>The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department's identified patient ages. Assist in management and care of lung nodule screening, staging, diagnostics, surgery, pre/post treatment, biomarker/molecular testing</p> <p>b. Understand nodule staging and create patient care plans</p> <p>c. Attend Tumor Board Meetings</p> <ol start="2"> <li>Maintain Incidental Nodule Program </li></ol> <p>a. Evaluate size and determine the correct plan of care</p> <ol start="3"> <li> <p>Smoking Cessation Counselor</p> </li><li> <p>Collaborate with STS Database</p> </li></ol> <p>a.
