We strive to honor and respect those living in our community through companionship and person-centered care while providing a comfortable and safe place to live. At Fitzsimons, we provide long-term care and short-term rehab and have a secured memory care neighborhood. We are conveniently located on the medical campus with UC Hospital and the Aurora VA Medical Center. Fitzsimons celebrates a low 14% staff turnover rate and substantial longevity among staff.</p> <p>Visit us online at: www.colorado.gov/cdhs/veteranshomes/fitzsimons</p> <p>Opt-in below to receive text message updates on CDHS recruiting events!</p> <p>CDHSCareers</p> <p> </p> <p>To provide basic patient/resident care under the direction of professional nursing staff and to provide restorative care for individuals who require acute and long-term care.</p> <p>Job responsibilities include, but are not limited to:</p> <ul> <li>Provide activities of daily living (occupational therapy) to include bed mobility, transfers, ambulation, dressing, performing personal hygiene, bathing, feeding programs, and toileting programs. </li><li>Provides occupational therapy/physical therapy/star programs as initiated by the Therapy Department and continued by the restorative aides. </li><li>Input notes in PCC on each resident to include accurate data on progress, use of equipment, therapies used, etc. </li><li>Performs restorative dining program for those residents requiring one-on-one attention during meals. </li><li>Monitor and inventory bed and chair alarms weekly. </li><li>Provides training to new and current staff on proper usage of adaptive equipment, splints/braces, lifts, and safety equipment. </li><li>Attends care plan meetings with residents, families, and key facility representatives </li><li>Attends monthly restorative meetings with the Restorative Director and Nurse Supervisors to review ongoing programs. </li><li>Attends skills lab and skills workshops to ensure all skills are maintained or updated as needed to maintain current CNA license. </li><li>Attends facility in-service trainings, all staff meetings, etc., as required. </li><li>Other Duties as Assigned </li></ul> <p>Special Qualifications: </p> <p>Must hold and maintain an active CNA certification in good standing from the Colorado Department of Regulatory Agencies, as well as a current BLS Healthcare Provider Card.</p> <p>Minimum Qualifications: </p> <p>High school diploma or General Education Diploma (GED)</p> <p>Substitutions:</p> <p>None.</p> <p>Preferred Qualification: </p> <ul> <li>Experience in a long-term care/assisted living environment </li><li>Experience transporting/driving residents </li><li>Excellent communication and customer service skills </li><li>Relevant State of Colorado experience </li></ul> <p>Conditions of Employment:</p> <ul> <li>Full Background CDHS employees (all Direct contact with vulnerable persons): </li></ul> <p>CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults - Mental Health Institutes, Regional Centers, Veterans Community Living Centers)</p> <ul> <li>TB Test/Flu Vaccine </li><li>Must be willing to obtain vaccinations, as required by the State and facility </li><li>Must possess a valid, unrestricted U.S. driver's license </li><li>May be required to travel/transport residents in state vehicles </li><li>Required to have a current, valid BLS Healthcare Provider Card </li><li>Shift Work: While every effort will be made for the schedule to reflect the shift/days agreed upon by my supervisor and me, I understand I may be scheduled on different days/shifts based on facility need. While leadership will ask for overtime volunteers as a first-line strategy to relieve staffing shortages, I understand that I may be required to work overtime as a part of this position. Shift work includes weekends, nights, holidays </li><li>May require on-call status, if needed, per the pay plan for this position </li><li>Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant's prior State employment.
