<p>Up to 3 months temporary position</p> <p>The Population Health Associate serves to collaborate with population health specialist in supporting primary and specialty care providers and practice support staff to improve population health performance and achieve quality incentives related to managed care contracts by managing and organizing data, engaging patients, and optimizing workflow, with the goal of optimizing our patients' health and well-being.</p> <p>Essential Job Functions:</p> <ul> <li>Extracts data from practice electronic medical record (EMR) or other internal databases to identify care gaps and assess trends in quality scores </li><li>In partnership with Population Health specialist, monitors, and assesses quality scores and performance related to screening, chronic disease management and outcomes for patients, providers and the network at large </li><li>Partnership with practice sites in identifying opportunities for pre-visit planning, reminders, and processes on the day of the visit, and loop closure related to tests and referrals following visits towards the aim of improved population health </li><li>Participates in meetings with accountable care organizations and payers as they relate to Population Health activities </li></ul> <p>Required Skills/Qualifications/Training/Experience:</p> <ul> <li>High School Diploma or Equivalent and experience in a health care setting </li><li>Experience with EMR to facilitate data abstraction and interpretation </li><li>Command of verbal and written English; positive interpersonal communication skills and good organizational skills </li><li>Must demonstrate integrity, sound judgment, and strong interpersonal skills </li></ul> <p>License / Certification: N/A</p> <p>Preferred Skills/Qualifications/Training/Experience:</p> <ul> <li>Knowledge of medical terminology </li><li>Understanding and experience with provider incentive programs </li><li>Previous experience in population health </li><li>College (Associate or Bachelor's) Degree </li></ul> <p>License/Certification: N/A</p> <ul> <li>Salary Range:$18.71-$28.26 </li></ul> <p>Sturdy Memorial Hospital is an equal employment opportunity employer.
