Succeed. #BeHere</p> <p>Location: US:WI:MILWAUKEE at our FROEDTERT HOSPITAL facility.</p> <p>This job is ON - SITE.</p> <p>FTE: 0.200000</p> <p>Shift: Shift 1</p> <p>Job Summary:</p> <p>This position as a Healthy Mindset Specialist will work to support our Intensive Cardiac Rehabilitation Program at Froedtert Hospital. This role will present a variety of healthy mindset group presentations to our patients on a rotating basis, as well as provide 1:1 coaching sessions.</p> <p>Considered a subject matter expert for the Pritikin Intensive Cardiac Rehabilitation (ICR) Healthy Mind-Set content. Responsibilities include conducting Pritikin ICR Healthy Mind-Set Workshops and consulting with physicians and ICR healthcare personnel to coordinate patient care needs. Duties also include providing 1:1 health-coaching/counseling sessions with program participants.</p> <p>EXPERIENCE DESCRIPTION:</p> <p>At least one year of experience in an outpatient, clinical setting performing psychological assessments, administering behavioral health interventions, and counseling with cardiac rehabilitation/secondary prevention or chronic disease.</p> <p>EDUCATION DESCRIPTION:</p> <p>Bachelor's Degree is required in at least one of the following areas: 1) Social Work 2) Counseling or 3) Psychology. In lieu of a Bachelor's degree in above specialties, Associate's or Bachelor's Degree in Nursing or a Bachelor's Degree in an Exercise Physiology-related field AND a wellness coach certification is required.</p> <p>TRAINING DESCRIPTION:</p> <p>Upon hire, candidate will undergo training in the provision of Pritikin Intensive Cardiac Rehabilitation services.</p> <p>SPECIAL SKILLS DESCRIPTION:</p> <p>Coaching, problem-solving, and motivational interviewing techniques.</p> <p>LICENSURE DESCRIPTION:</p> <p>Current Wisconsin licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) or current Wisconsin Licensure as a Registered Nurse AND Wellness or Health Coaching certification endorsed by organizations such as NSHC, ACE, and Wellcoaches.
