New competitive salaries, immediate opening, generous time off packages, 401K + match, and so much more!</p> <p>Qualifications:</p> <ul> <li>Possession of a current license to practice as a registered professional nurse or the equivalent license and education to practice as a healthcare professional in the practicing state </li><li>Minimum 3-5 years of nursing experience, with at least 2 years in hospice, palliative care, or related specialty </li><li>Previous supervisory or charge nurse experience in healthcare, homecare or hospice </li><li>Strong knowledge of end-of-life care practices and symptom management </li><li>Demonstrated experience with electronic medical records, HCHB experience strongly preferred </li><li>Proficiency with Microsoft Office Suite and financial software systems </li><li>Excellent organizational and time management skills with ability to manage multiple priorities </li><li>Strong analytical and problem-solving abilities </li><li>Exceptional attention to detail and accuracy in documentation </li><li>Excellent communication and interpersonal skills </li><li>Ability to work independently and make sound clinical and operational decisions </li><li>Valid driver's license and proof of insurance is required </li><li>Must possess current CPR Certification </li></ul> <p>HOURS: (3) 12-hour shifts/week 12PM-12AM - Including every other Saturday. On Call 3 Holidays/Year.</p> <p>LOCATION: Barrington, IL.</p> <p>What We offer:</p> <ul> <li>Great culture and team atmosphere </li><li>Comprehensive benefits, including medical, dental, and vision, effective on the first of the month </li><li>401(k) retirement plan with a generous company match </li><li>Generous time off accruals </li><li>Paid holidays </li><li>Tuition Reimbursement </li><li>Employee Referral Program </li><li>Hospice Certification benefit and salary increase </li><li>Merit Increases </li><li>Employee Discount Programs </li></ul> <p>What You'll Do:</p> <ul> <li>Oversee EMR functions, including SOC, POC, CTIs, HOPE documentation, and management coordination notes. </li><li>Review and approve care plan changes, medication orders, and aide care plan updates. </li><li>Process HEAs and ensure accurate location settings. </li><li>Collaborate with Quality and CTAs to remove holds. </li><li>Complete weekly RN notes for IDG meetings. </li><li>Review charts for GIP appropriateness and coordinate with social workers, field staff, and the medical director. </li><li>Document comprehensive hospice narratives for all direct admissions. </li><li>Develop and maintain daily/weekly schedules for nursing, CNA, physician, and support staff. </li><li>Manage call-ins, shift trades, agency staffing, and weekend/MD coverage. </li><li>Participate in hiring processes, orientation, annual reviews, competencies, TB testing, and tracer documentation. </li><li>Order, organize, and reconcile supplies across multiple vendors. </li><li>Complete ACHC audit tasks. </li><li>Collaborate with Facilities team on oxygen supply issues, porter services, and maintenance requests. </li><li>Performs other duties as assigned </li></ul> <p>Text 10466 to 847-416-8078 to APPLY!</p>
