You'll ensure patients receive coordinated, personalized behavioral health services while fostering a high-performing, mission-aligned care team.</p> <p>Key Responsibilities:</p> <ul> <li> <p>Lead the delivery of diagnosis-specific behavioral health care in the home</p> </li><li> <p>Manage patient care planning, staffing, and coordination across providers and facilities</p> </li><li> <p>Oversee timely initiation of care in compliance with all regulations</p> </li><li> <p>Approve and update care plans and physician orders</p> </li><li> <p>Guide clinical staff through regular communication, performance oversight, and coaching</p> </li><li> <p>Support QAPI (Quality Assurance and Performance Improvement) initiatives to enhance outcomes</p> </li><li> <p>----------------------------------------------------------------------------------------------</p> </li></ul> <p>What You Bring</p> <p>Education:</p> <ul> <li> <p>Associate's Degree in Nursing (required)</p> </li><li> <p>Bachelor's in Nursing or related field (preferred)</p> </li></ul> <p>Experience:</p> <ul> <li> <p>Minimum 2 years as a Registered Nurse in behavioral health or home health</p> </li><li> <p>At least 1 year in a leadership or supervisory role</p> </li></ul> <p>Licensure:</p> <ul> <li>Current and active RN license in the state of practice </li></ul> <p>Tech Savvy:</p> <ul> <li> <p>Experience with clinical enterprise systems (e.g., HomeCare HomeBase, Workday)</p> </li><li> <p>Proficient in Microsoft Office Suite (Word, Excel, Outlook)</p> </li><li> <p>----------------------------------------------------------------------------------------------</p> </li></ul> <p>We're Building Something Great - Lead the Way.</p> <p>At Elara Caring, leadership isn't about control-it's about empowering others.
