In collaboration with other clinical disciplines, this role integrates these evidence-based practices to facilitate optimal coordination of care, patient, family and clinical staff communication and education, and optimal transition/discharge planning in accordance with CMS (Medicare's) Condition of Participation. The RN Case Manager serves as an expert in coordinating the patient care trajectory though the continuum of care and that the patient is at the right level of care that is appropriate.
