<p>Overview</p> <p>Trellis Services is hiring abd Adult Life Planner who will provide information and resources to support an individual accessing Autism Waiver Services and their family with transition planning as a means to successfully transition from In- Home Support Services/School Services into Adult Services.</p> <p>Responsibilities</p> <ul> <li>Identify client skills related to independence, self-advocacy, self-direction and employment options. </li><li>Assist in connecting the family and client to support services and resources to help guide them into appropriate adult programs, volunteer opportunities and/or employment. </li><li>Ensure the development of an Adult Life Planning Treatment Plan to guide services. </li><li>Complete mandatory trainings and attend required meetings. </li><li>Document services delivered accurately and timely according to company policies. </li><li>Report clinical and scheduling concerns promptly. </li><li>Provide clinical and professional feedback to staff. </li><li>Adhere to all company compliance policies, written or unwritten. </li><li>Complete other tasks as assigned by supervisor. </li></ul> <p>Qualifications</p> <ul> <li>Master's Degree </li><li>5 Years' Experience in Special Education Services </li><li>Awareness of various Adult Services - how to connect to services provided by the Developmental Disability Administration, Division of Rehabilitation Services, Department of Labor, Licensing and Regulation, Mental Health Administration/Behavioral Health Administration and Social Security Administration </li><li>Awareness of local DDA funded adult service agencies </li><li>Awareness of how to apply for and connect to disability support services at the post-secondary level </li><li>Awareness of local community support groups </li><li>Awareness of needed documents for long term financial support and decision making, and planning for adulthood including Maryland ABLE and Guardianship and the Alternatives to Guardianship </li><li>Communication Proficiency </li><li>Customer/Client Relationship Focused </li><li>Business Acumen </li><li>Cultural Alignment </li><li>Technical Proficiency </li><li>Ability to multitask and maintain thoroughness </li><li>Thrive in a fast-paced environment </li><li>Adaptable to change </li><li>Positive approach to daily routine </li></ul> <p>Trellis is part of LEARN Behavioral, a national organization dedicated to nurturing the unique potential of children with autism and special needs.
