Job Description Elwyn seeks a Quality Improvement (QI) Specialist to conduct internal evaluations of services to ensure quality care which fulfills all regulatory, funding agency and internal standards. Perform assessments of all services and make recommendations for improvement in all aspects of service delivery. Develop and implement policies for assuring compliance with all fire, health, safety and security regulations. Participate in on call responsibilities. DUTIES AND RESPONSIBILITIES: Conduct internal evaluations as part of the Supports for Living (SFL) Continuous Quality Improvement (CQI) program. Oversee implementation of plans of correction as assigned Self-initiate Quality Improvement (QI) and Quality Assurance projects to aid in Elwyn's on-going commitment to the delivery of exceptional services Participate in the development of policies and procedures, utilization management, facility safety and fire safety Attend committees and sub-committees as assigned Report Quality Improvement needs to the Director of QI Anticipate future needs or problem areas and suggest effective action Conduct reviews/audits of program plans and related documents for compliance with professional quality and regulatory standards Conduct chart audits Conduct interviews with individuals supported, their families, staff members, advocates, etc. to verify quality services are being delivered Evaluate all aspects of programming and ensure all policies and procedures are followed and enforced Provide in-service trainings for all SFL staff to support compliance, program development and thorough understanding of all regulations Act as a resource to program staff for all questions regarding forms, policies, and protocols in regards to the annual program plan writing, implementation, revision, and training Maintain a current file of all regulatory standards and updates Maintain a current manual for Program Specialists and Qualified Intellectual Disabilities Professionals Maintain records of all compliance documentation and survey results per policy Assist in preparing for surveys and hosting survey teams Review, update and maintain a current file of all QI tools used as part of the CQI, including but not limited to tools for chart audit, medication audit, physical site audit, finance audit, meal observations, data audits Maintain up to date knowledge of all regulations through participation in external training offerings and review of regulatory body communications, communicate changes to the program, and keep all programming policies and procedures current Evaluate services to determine their meeting of customer needs Obtain and maintain current Certified Investigator training and credentialing Complete investigations of incidents as assigned Enter data concerning incidents into current incident management systems and retrieve information as needed Complete Peer Reviews of investigations Coordinate incident management as assigned Perform other duties as assigned EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS: Must be Qualified Intellectual Disabilities Professional (QIDP) qualified: (a) At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities AND (b) Is either a physician and surgeon, or a osteopathy physician and surgeon, a registered nurse, or a human services professional possessing at least a bachelor's degree in a human services field, including but not limited to sociology, special education, rehabilitation counseling, psychology, all professional disciplines (e.g., Occupational Therapist, Physical Therapist, physical therapy assistant, speech-language pathologist, audiologist), academic disciplines associated with the study of human behavior (e.g., speech communication, gerontology), human skill development (e.g., counseling, human development), humans and their cultural behavior (e.g. anthropology) or study of human condition such as literature, the arts Master's degree in aforementioned discipline or related field preferred Demonstrated ability to work effectively as part of a team and in leading a team through a quality improvement process Must have highly developed organizational skills Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint; report-writer experience preferred Obtain and maintain Pennsylvania Certified Investigator certification Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record Equal Opportunity Employer Elwyn is an Equal Opportunity Employer.
