<p>Title: Licensed Practical Nurse Position Reports to: Site Nursing Management</p> <p>Department: As Assigned Employment Status: Non-exempt</p> <p>Job Grade: Grade 10 Revised: December 2021</p> <p>General Responsibilities:</p> <p>We provide culturally competent care to patients from birth through late adulthood, utilizing a multidisciplinary team approach. As a Licensed Practical Nurse (LPN) you are an essential member of the team delivering care in areas such as family medicine, primary care, pediatrics, obstetrical and gynecological care, medication assisted treatment, trans and behavioral health.</p> <p>Under the direction of a Registered Nurse, your responsibilities include, but are not limited to ensuring a timely response to and management of incoming patient calls and directing telephone encounters to the appropriate staff for resolution or bringing calls to a close within your scope of practice, providing direct patient care, vaccine administration and monitoring, and following standardized procedures according to established workflows. As an LPN you are responsible for all care provided and act as a resource person for medical assistants and other team members.</p> <p>Required Qualifications:</p> <ul> <li>Current State of Rhode Island Licensed Practical Nurse license </li><li>One or more years' experience in a health care setting preferred, new LPN graduates will be considered </li><li>Ambulatory Health Care experience preferred </li><li>Ability to communicate effectively and work collaboratively with patients, providers, other members of the healthcare team, other disciplines, and community agencies </li><li>Professionalism, self-starter </li><li>Ability to document clearly and concisely in the EMR </li><li>CPR certification required </li></ul> <p>Physical Effort/Environment:</p> <p>Moderate physical activity, walking, standing, stooping, and occasional lifting. Potential exposure to blood-borne pathogens, biohazardous materials, and infectious diseases (i.e. tuberculosis, COVID-19). Manual dexterity required for injections, immunizations, and computer use.</p> <p>Work Schedule Demands:</p> <p>As scheduled during hours of operation (inclusive of weekend and evening hours as assigned) with occasional unscheduled overtime.</p> <p>Communication Skills:</p> <p>Have sensitivity to the patient population we serve, by identifying cultural, social, and economic characteristics. Applies verbal and non-verbal communication skills in patient interactions and utilizes teach-back, active listening, written materials, and visual aids.</p> <p>Able to document clearly, competently, and concisely in the medical record. Ability to maintain confidentiality of information and communicate in a professional manner with patients, families, other staff, and outside contacts.</p> <p>Confidentiality of Information:</p> <p>Maintains confidentiality and compliance with HIPAA at all levels of patient interaction and when in the patient's medical record.</p> <p>Significant Job Functions:</p> <p>It includes promotion, maintenance, and restoration of health and utilizes standardized procedures leading to predictable outcomes that are in accord with the professional nurse regimen under the direction of a registered nurse. In situations where registered nurses are not employed, the licensed practical nurse functions under the direction of a licensed physician, dentist, podiatrist, or other licensed healthcare providers authorized by law to prescribe. Each L.P.N. is responsible for the nursing care rendered.</p> <ul> <li>Safely practice as an LPN under the direction of a Registered Nurse licensed physician, or other licensed healthcare providers authorized by law to prescribe. </li><li>Ability to utilize standardized workflows in carrying out the responsibilities of your position </li><li>Appropriately use medical terminology in documentation. </li><li>Perform proactive chart review in anticipation of provider and patient needs and huddle with provider/MA as needed </li><li>Ensure the safe delivery of care to patients (i.e. newborn, infant, child, adolescent, adult, geriatric) </li><li>Assist with nurse line phone calls and when calls are within scope of practice bring calls to a resolution or assign to the appropriate healthcare team member. </li><li>Use available resources and references in the management of patient care </li><li>Under the direction of an RN execute patient visits and provide evidence-based health education and information at an appropriate level of understanding for the patient </li><li>Administer vaccines and other injections as ordered by a provider and in accordance with agency procedures and policies </li><li>Support various population needs, including trans health and mental health as part of primary care </li><li>Accurately obtain and document vital signs (i.e. weight, blood pressure, BMI, pain assessment, temperature) </li><li>Review externally ordered labs and diagnostic imaging and notify provider of abnormal results according to company policies and workflows </li><li>Assess patients for potential communicable diseases and utilize infection control precautions. (i.e. isolation of patient with TB or Chicken Pox) </li><li>Act as a resource for other staff </li><li>Notify patients of test results under the guidance of an RN or Licensed Providers as described above and assess patient understanding </li><li>Receive and verify verbal orders from providers, assuring accuracy of the order </li><li>Maintain line of communication with supervisor, providers, and other staff members </li><li>Actively participates in performance improvement activities </li><li>Respond to codes and rapid responses </li><li>Utilize standing orders and SOP's </li><li>Maintain OSHA and HIPPA guidelines </li><li>Adhere to organizational policies and procedures </li><li>Utilize electronic medical record alerts and act on them when appropriate </li><li>Maintain standard precautions </li><li>Assume responsibility for professional continuing education </li><li>Perform other tests, duties and special projects as assigned </li><li>Utilize training resources including the nursing manual, workflows, and instructional materials to perform at top of license </li></ul>
