With innovative technology solutions and a dedicated, experienced team throughout the state, we're changing the face of foster care and positively impacting children and families' lives for generations to come.</p> <p>Join our team to continue to do good and create history serving Florida's children and families!</p> <p>The Healthy Families (HF) Mental Health Counselor provides in-depth, in-home assessment and counseling information and services to HF program participants, with the goal to promote recovery and overall wellbeing. With a focus on meeting program outcome goals while upholding the CHS Standard of Care, the incumbent assesses and identifies client and family needs; as well as plans, coordinates, provides and evaluates necessary services and treatment. Overall, the HF Mental Health Counselor contributes to the CHS high performance culture by exhibiting our values and providing quality results that position CHS as the leader in delivering proactive behavioral health, case management, community and early childhood solutions for children and families.</p> <p>WHY JOIN CHS?</p> <ul> <li>Uplifting mission-driven work culture </li><li>Make an impact in your community and become a part of Florida's history! </li><li>Growth and professional development opportunities </li><li>Great benefits package, including generous paid time off and holidays </li><li>Opportunity to make a positive difference in children and families within your own community! </li></ul> <p>Primary Job Functions</p> <ol> <li>Provide in-depth, in-home assessment and counseling information and services in the areas of substance abuse, domestic violence and mental health to HF program participants, with the goal to promote recovery and overall wellbeing. </li></ol> <p>Direct Counseling Services (60%)</p> <ul> <li>Collect referrals for family specialist services and determine eligibility based on identified risk factors. </li><li>Deliver evidence-based treatment interventions (e.g., Trauma-Focused Care, Infant Mental Health), working with families with challenging mental health issues. </li><li>Provide in-depth clinical psychosocial assessment, diagnosis of clinical conditions, treatment plans and interventions. </li><li>Facilitate psycho-educational classes and/or groups such as parenting skills, independent living skills, anger management, behavior management, substance abuse, MAPP/Pride training, etc., as needed. </li><li>Provide case management services including information, plan referral and linkages, as needed. </li><li>Oversee the behavioral program or treatment interventions associated with the client's treatment plan. </li><li>Coordinate and supervise therapeutic family visitation, as needed. </li><li>Refer clients for specialized services and follow-up with family and service providers on those services. </li><li>Coordinate services with other professionals; liaise with other social service agencies and organizations, as needed. </li><li>Meet established program productivity goals while upholding the CHS standard of care. </li></ul> <p>Family Engagement & Services Support (30%)</p> <ul> <li>Accompany the Family Support Specialist (FSS) on home visits, as appropriate. </li><li>Participate in family service planning and in meetings between the FSS and their supervisor, as appropriate. </li><li>Review FSS other team member home visiting and supervision records. </li><li>Participate in various HF-related conference calls and convey information to other team members. </li><li>Provide consultations; lead and facilitate staff briefings and meetings; </li><li>Conduct regular meetings to review referral process, caseload and family progress. </li><li>Train team members on topics related to infant health or other individualized treatment plan components, as needed. </li></ul> <p>Program Quality & Reporting (10%)</p> <ul> <li>Collect, enter and maintain accurate data for services provided, including referrals given, referrals made and outcomes, into the Healthy Families Florida (HFF) data system by the 10th of each month; perform monthly QA reviews. </li><li>Follow the HFF policies, procedures and data collection guidelines; review and monitor clinical billing, as needed. </li><li>Participate in the peer review process to oversee that services provided comply with contract, Medicaid and licensing requirements, COA, and internal quality assurance standards, as needed. </li><li>Distribute the Ounce of Prevention/HFF participant satisfaction survey to all enrolled families during the month of April. </li><li>Attend the HFF Family Engagement and Support Specialist core training within 90 days of hire. </li><li>Attend the HFF Domestic Violence, Substance Abuse and Child Abuse & Neglect training, within the HFF timeline guidelines. </li></ul> <ol start="2"> <li>Contribute to a positive, engaging work environment. </li></ol> <ul> <li>Develop a strong knowledge base and stay current on HSS-related issues and trends. </li><li>Participate actively in departmental meetings, training and education, as well as the quality management process. </li><li>Comply with CHS's code of conduct, policies, practices, contracts and other legal obligations. </li><li>Assist with training other team members and providing back up when necessary. </li><li>Pick up projects on the fly; perform other duties as assigned from time to time. </li><li>Demonstrate the CHS Common Bond Values in the performance of all job duties. </li></ul> <p>Job Qualifications</p> <p>Education, Licenses & Certifications:</p> <ul> <li>MA/MS from an accredited university/college in Social Work, Psychology, Marriage & Family Therapy or Mental Health Counseling - OR -MA/MS from an accredited university/college with a major in the field of Nursing, Rehabilitation, Special Education, Health Education or a related human services field AND have completed graduate level coursework in at least 4 of the following 13 content areas: human growth and development, diagnosis and treatment of psychopathology, human sexuality, counseling theories and techniques, group theories and practice, dynamics of marriage and family systems, individual evaluation and assessment, career and lifestyle assessment; research and program evaluation, personality theories, social and cultural foundations, counseling in the community settings, and substance use disorders. </li><li>Active, unrestricted State of Florida clinical license in Mental Health, Clinical Social Work, Marriage & Family Therapy or Psychology (LMHC, LCSW, LMFT). </li><li>FL Drivers' License within 30 days/hire, along with daily access to a reliable and insured vehicle. </li></ul> <p>Experience:</p> <ul> <li>Minimum of 3 years of post-Bachelor's experience working with at-risk children and families, required. </li><li>Experience providing therapy to at-risk children and families, strongly preferred. </li></ul> <p>Competencies</p> <p>Knowledge of:</p> <ul> <li>COMMUNITY RESOURCES | COMPREHENSIVE THERAPY WITH CHILDREN AND FAMILIES AFFECTED BY ABUSE, NEGLECT OR SUBSTANCE ABUSE </li></ul> <p>Skills and Proficiency in:</p> <ul> <li>Planning, organization, time management and oral/written communication. </li><li>Interpersonal relationship building, collaboration, teaming and conflict resolution. </li><li>Consulting, facilitation, coaching and mentoring </li><li>Software including MS Excel, Outlook, Word and data entry systems. </li></ul> <p>Ability to:</p> <ul> <li>Perform at a high level of autonomy with general supervision under strong demands in fast-paced, diverse, sometimes ambiguous environments. </li><li>Communicate and interact effectively with team members and clients from diverse backgrounds. </li><li>Set appropriate limits and boundaries with clients;
