Serves as an advocate to ensure patient's rights are protected with patients, families and team members. </li></ul> <p>What do I need to be considered?</p> <ul> <li>STAR mentality (Selfless, Tough, Accountable, Respectful). </li><li>Bachelor's degree in healthcare related field preferred. </li><li>Knowledge of the social service needs of the aging population. </li><li>At least two years of experience in Social Work, Case Management or related field. </li><li>At least two years of experience performing social services, discharge planning or care transitions in a post-acute or acute care setting preferred. </li></ul> <p>About Hillcrest:</p> <p>As a local organization, we offer a tight-knit, friendly environment with the benefits and options of a large company!</p>
