<p>Crossroads Health, a 501(c)(3) non-profit organization with facilities located in Mentor, Painesville, and Cleveland, serves Northeast Ohio communities with comprehensive behavioral and primary integrated healthcare, early childhood services, extended housing and recovery services-no matter one's ability to pay. Our mission is to provide hope, healing, and healthcare to everyone.</p> <p>Our Intake and Access Team is currently looking for an Access (Intake) Counselor to join the team. This position is responsible for assessment, diagnosis, and referral across youth, adolescents, and adults. This licensed mental health provider will engage as a member of a collaborative team.</p> <ul> <li></li><li>Level 1: Bachelor's Level LSW </li><li>Level 2: LPC/LSW/MFT </li><li>Level 3: LPCC/LISW/IMFT </li></ul> <p>More specifically, this position is responsible for:</p> <ul> <li>Providing trauma informed assessments to clients and their family system in the office. </li><li>Providing accurate, concise, and concurrent documentation according to program procedures and policies. Including, but not limited to Diagnostic Assessment, diagnosis, treatment plans, progress notes and other intake documents. </li><li>Interviewing all new clients to conduct thorough diagnostic assessments. </li><li>Providing and coordinating crisis intervention and triage services. </li><li>Managing client records in accordance with Ohio MHAS, Joint Commission standards and Crossroads Health's case record procedures in a timely, professional, accurate, and legible manner </li><li>Developing and maintaining positive relationships with referral sources and community providers </li></ul> <p>Position Requirements:</p> <ul> <li>Bachelor's Degree in Social Work </li><li>Master's degree in Social Work/Mental Health or related field (preferred) </li><li>LPC/LSW/MFT required. LPCC/LISW preferred. </li><li>Experience performing Diagnostic Assessments in a mental health setting (preferred) </li><li>Experience in chemical dependency assessment (preferred) </li><li>Experience in crisis intervention, triage, and knowledge of community resources (preferred) </li><li>Provide consultation and collaboration with community partners and clients' treatment teams. </li></ul> <p>Our organization is committed to equal employment opportunity.
