Monitors practices for effective implementation </li></ul> <p>and achievement of results.</p> <ul> <li>Conducts resident assessments, suggesting level of needed service packages to residents and their families, as </li></ul> <p>directed.</p> <ul> <li>Ensures delivery of compassionate quality care evidenced by adequate services and staff coverage for the facility, </li></ul> <p>appropriateness of staff, absence of odors, cleanliness, absence of pressure wounds, and apparent maintenance of</p> <p>optimal physical, mental and, psychosocial function.</p> <ul> <li>Demonstrates knowledge of and application of Key Clinical Quality Indicators. Proactively monitors and implements </li></ul> <p>systems to achieve and/or surpass company thresholds.</p> <ul> <li>Reviews 24-hour report from every unit daily to ensure timely, effective responses to significant changes in condition, </li></ul> <p>transfers, discharges, use of physical or chemical restraints, unexplained injuries, potential abuse or neglect, medication</p> <p>errors, loss of resident property, or any evidence of resident or family dissatisfaction, as directed.</p> <ul> <li>Gathers and analyzes facts, assesses situations, develops, and implements appropriate actions to investigate, notify </li></ul> <p>appropriate parties, resolve issues, record facts properly, and prevent future reoccurrence. Communicates</p> <p>findings/results to the Wellness Director or Executive Director.</p> <p>Education and Experience:</p> <ul> <li>Graduate of an accredited school of nursing or where states allow, minimum of 2-5 years leading a care team. </li><li>Must be licensed as an LPN, and must maintain current and unencumbered licensure </li><li>Prefer two-year professional nursing experience in a senior living care setting. </li><li>Two-years supervisory/management experience with direct care staff. </li></ul>
