<p>We are so glad you are interested in joining Sutter Health!</p> <p>Organization:</p> <p>SCAH-Sutter Care at Home - Bay</p> <p>Position Overview:</p> <p>The role of the Clinical Referral Specialist is to act as the in-office clinical liaison that represents the SCAH agency via telephone receiving and processing referrals for patient care in the regional home health referral management team. Referral source interactions include, but are not limited to working closely with referring hospitals, physicians, board and care facilities, skilled nursing facilities, patients, families, and the general public. Primary clinical responsibilities include receiving/processing referrals and ensuring that admissions occur in a timely and consistent manner. The Clinical Referral Specialist enters referral information into the EMR and creates initial orders to facilitate the start of care. As the Clinical Referral Specialist, this person frequently interacts with referral sources, other members of the referral center staff, home health, AIM and hospice staff across SCAH offices and all others necessary to facilitate the start of patient care. Consistently utilizes and promotes use of excellent customer services skills. The Clinical Referral Specialist position reports directly to the Home Health Administrator</p> <p>Job Description:</p> <p>EDUCATION</p> <ul> <li>Equivalent experience will be accepted in lieu of the required degree or diploma. </li><li>Bachelor's: Nursing or health related field preferred. </li></ul> <p>CERTIFICATION & LICENSURE</p> <ul> <li>RN-Registered Nurse of California </li><li>DL-Valid Drivers License Class C or B </li><li>AUTO-Automobile Insurance </li><li>BLS-Basic Life Support Healthcare Provider </li></ul> <p>PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:</p> <ul> <li>1 year of professional experience in an acute or similar setting. </li><li>1 year of experience in a home health position strongly preferred. </li><li>1 year of direct patient care experience in a home health preferred. </li><li>Experience in coordinating/processing home care referrals </li></ul> <p>SKILLS AND KNOWLEDGE</p> <ul> <li>Must have the clinical knowledge and critical thinking skills to effectively plan and provide coordination of patient care consistent with standards and regulations </li><li>Must have exceptional interpersonal and customer services skills </li><li>Must be able to effectively solve unique problems as they arise or identify when to consult the supervisor </li><li>Must have knowledge of current Hospice/Home Health admission criteria; Medicare, Medi-Cal, Commercial, State and Federal regulations </li><li>Familiarity with ICD-10 coding preferred but not essential </li><li>Must be able to demonstrate problem-solving abilities as well as excellent telephone, interpersonal, verbal and written communication skills in English </li><li>Strong commitment to teamwork and customer service, both internally and externally, is required </li><li>Must have proven strong/proven data entry and general computer skills </li><li>Must be able to demonstrate proficiency (after training and introductory period) in efficient use of electronic medical record systems </li><li>Must be able to flex with the increased workflow when census is higher and recognize urgency of each task </li><li>Must be flexible with schedule including, but not limited to, the ability to participate in department rotation for weekend coverage if needed </li><li>Must be able to drive to designated SCAH location(s) for meetings, training, and needs as designated by business needs </li></ul> <p>Job Shift:</p> <p>Days</p> <p>Schedule:</p> <p>Full Time</p> <p>Shift Hours:</p> <p>8</p> <p>Days of the Week:</p> <p>Monday - Friday</p> <p>Weekend Requirements:</p> <p>As Needed</p> <p>Benefits:</p> <p>Yes</p> <p>Unions:</p> <p>Yes</p> <p>Position Status:</p> <p>Non-Exempt</p> <p>Weekly Hours:</p> <p>40</p> <p>Employee Status:</p> <p>Regular</p> <p>Sutter Health is an equal opportunity employer EOE/M/F/Disability/Veterans.</p> <p>Pay Range is $58.68 to $78.86 / hour</p> <p>The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate's experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
